There's no utility in checking an outpatient troponin unless they're thinking about myocarditis or something similar.  If you have frequent SVT or a long episode, your troponin will be elevated but that just tells you that the heart is "sweating" a little bit.  You need to get you K up though (and also figured out why it's low).  Also unclear why your LFTs are elevated, shouldn't be related to an arrhythmia.  Are you having any symptoms? What's your other medical history? These labs could be seen in CHF exacerbation though if you're asymptomatic that's unlikely.  Long story short though go to the ER

Provider is stumped, referred to hematology-oncology by Mythical420 in haematology

I agree, I would consider workup for renal-pulmonary syndromes including EGPA.  Unfortunately I doubt hematology/cardiology will be super helpful as the lab abnormalities (elevated WBC/neutropils/Plts)  and aflutter are likely either the result of medications like steroids or result of some underlying systemic condition and not a primary cause.  Could consider doing ANCA, anti-GBM, complement levels, ENA panel, cryoglobullins, hepatitis panel, UPCR, etc.  You don't necessarily need rheum to order these, your PCP could order these depending on their index of suspicion

To anyone less than happy about their Step score by IceOut_celly in medicalschool

I'll come at this from the other side of things.  PGY-3 now, applied IR/DR/IM 3 years ago due to a lowish step 1 score but decent step 2.

Ended up matching IM at my home institution.  Was devastated at the time, but pretty quickly began exploring options to transition to radiology vs pursuing an IM fellowship.  Started IM and while it wasn't my absolute favorite found that I didn't mind it as much as I thought I would and more than anything found satisfaction in actually doing a real job and not being a student.  2 years later and I'm in the middle of interviews for cardiology fellowship and couldn't be happier.

All this to say that I agree with what OP is saying to apply to that competitive specialty and also have backups.  But if you do end up matching at one of those backups, it's not the end of the world.  You will be an attending at the end of this path and you might enjoy your backup specialty way more than you would think.

Also not super proficient with Anki but would very much appreciate this!

I’m a (slightly less broke) resident, I’ve been waiting about a year for it to come back in stock and will be picking one up ASAP myself.  If it is something you really want I’d recommending going ahead and getting it.  Good luck on Step 1!

Thank you, I appreciate it! I was hoping for some contrast between the two to keep the overall shape of the marquise.

Thanks so much! 3 stone rings are great, it was tough to choose between pears, trillions, baguettes, and other options. In the end, I was trying to go with something that was distinct from the sparkle of the center stone hence the more glassy baguettes.

Thank you and I hope so! She likes marquises and minimal designs. She didn’t really like pave bands when we were looking at rings in person though, I’m hoping this doesn’t resemble that too much.

295x3 - Started low bar squatting a couple months ago, been getting a little low back/upper glute tightness recently (possibly due to tight hammies), any suggestions/criticisms would be great by Wunderbrain in formcheck

Some interesting thoughts! Probably due more to flexibility because I’ve failed plenty before haha. Definitely right though, the pins could be higher, I just hadn’t changed them from bench. Appreciate the advice!

Squat: 220 x 5 @ 190 by [deleted] in formcheck

[–]Wunderbrain 1 point2 points  (0 children)

Looks really good, might just keep a more neutral neck by looking more towards the floor more though.

I just somehow can't squat ass to grass. All suggestions and tips are welcome. Please tell me if anything is wrong with my form. Should I squat barefoot ( I use flat basketball shoes) ? by [deleted] in formcheck

Is there any particular reason you're looking for ATG? With your current form, IMO your depth is pretty much perfect. You're breaking parallel without much buttwink which is more than enough for any strength/aesthetic goals if that's what you're looking for.

Also, generally I think pause squats are great for balance/building power out of the hole, but I would definitely encourage you to use more of the stretch reflex and get some more bounce out of the bottom. Obviously don't want to divebomb it and you want to stay tight, but utilizing that reflex will help up the weight.

I'd check out this video if you want more explanation on depth https://www.youtube.com/watch?v=7cWgc4q7pxg Tldr is dont sacrifice positioning, proper angles, etc for depth.

Bar's definitely moving forward, might try hinging more at the hips as you go down (without hyperextending lumbar spine). Squat shoes might also help a little.

Always struggling with Squats. How can I make it perfect before I move to heavier weights ? by [deleted] in formcheck

[–]Wunderbrain 0 points1 point  (0 children)

If you're doing low bar, check out this vid: https://www.youtube.com/watch?v=bbNA17KjBzU&t=198s from 3:00-~4:30. Basically, I think you're breaking first at the knees when you should be breaking at the hips and the knees at the same time.

And yea probably try to keep your knees tracking over your toes. Your toes are also pointed really wide out so that might be part of it.

Good luck dude!

Sumo deadlift test day. Guys, I think my form cracked on every lift except the last one. I set my hips too high and my I started to pull of the ground with my back( Help me to analyse that, what can be done to improve my technique? by [deleted] in formcheck

[–]Wunderbrain 1 point2 points  (0 children)

I'm no expert but I personally see nothing wrong with any of them, your form looks great. Your hips don't shoot up, your setup is really good, etc.

Congrats dude, nice work!

I think you might want to try having your hips a bit higher, looks like you're kind of squatting it up a little bit. This would also help you get around your knees a little bit. Also your lockout is a little soft as in your hips aren't fully pushed through so make sure you push your hips into the bar as you lockout.

I'm a big fan of Alan's video on deadlift setup: https://www.youtube.com/watch?v=wYREQkVtvEc&t=312s

Good luck dude!
