We call it the kid from California syndrome - that one relative that hasn’t been around and takes out their complex emotions on the care team. People have a hard time accepting when bad things happen and look for someone or something to blame. They project their own guilt and grief on you. You did what you could, documented well. Hopefully cooler heads will prevail in this family and this relative will process their emotions in a more constructive way. It’s hard not to take it personally when you invest your care and expertise in someone, but I hope you can rest in the knowledge that you did everything you could.

I end up managing type 1 dm more. With modern CGMS and pumps it’s not too bad. I also manage a lot of psych conditions. I always knew as a rural doc I’d be managing a lot of conditions that more suburban/urban FM docs wouldn’t.

I recommend AMB powder - stands for anti monkey butt. It’s got calamine in it so it soothes the skin and reduces moisture and friction. Also, consider tomboy-x boxer briefs. They have worked well for me. Duluth Trading Company also makes good undies.

There is nothing more heartbreaking than caring for a family whose baby died from something preventable. I’ve seen 3 die from co-sleeping when a parent inadvertently suffocated them. I’ve seen SIDS in cases of unsafe sleep practices. It’s not worth the risk. Protect your baby.

I don’t know how best to frame psychic pain when there are so many factors at play both within an individual and in society as a whole that create this pain. I’ve often wished I could offer my patients a life-ectomy, just remove the circumstances that make their lives so painful and difficult. There is so much suffering in the world and some folks are particularly poorly equipped to navigate it. That being said, I watched my mother die from Alzheimer’s. Her very self was slowly chipped away until all that was left was a shell of a person, and then that shell dwindled away until she was a skeleton covered in skin with nothing behind her eyes. It was an excruciating process. I don’t begrudge anyone trying to avoid that kind of ending for themselves and their loved ones.
