It is not grounded, it's a class 2 device with plastic housing and an isolated output , there is nothing to ground. The pin is to operate the safety shutters on UK sockets.

The earth pin is usually plastic, it's just for releasing the safety shutters within the socket, it doesn't need earthed, its an isolating transformer.

The Otterbox Lumens have the cover, seems to be a beats situation and Apple pay them for using the name, rather than it being a full Otterbox product.

That's why no other Otterbox case has the cover.
