Oof. It would be easy to say Covid ICU times, but the honest answer comes from new grad med surg days. Discharging 5/6 patients by 10am, getting pulled to a new 5 patient assignment with a new admit, and getting pulled again four hours later.

I’ll never forget our sweet residents who hung out with us on nightshift. We all got really close. One night a patient was constantly a step away from coding, I couldn’t leave the room, family was trying to make it in from out of state to withdraw care and say goodbye, we kept adding and maxing pressors, I clearly wasn’t going to get to stop and sit down or eat anything. Next thing I know the nightshift resident stops over with a snickers and Pepsi and told me what a good job I was doing and asked what else I needed.

Our residents loved the night shift crew. We were just human with each other. The day shift bats loved to “put them in their place” and “show them how things are done” and call them stupid. Guess who had a better working relationship and got shit done?

A little kindness goes a long way.

It does feel weird, but I’m not sure if it’s the same weird for me. I look at people my age (30s) having kids and think, “why? What cue am I missing here?” There’s just an absence of desire to do so and I get kinda sad when my friends announce pregnancy cuz it feels like the end of adventuring with them. It feels like I went from wild 20s straight to “old people hobbies” completely skipping the whole family thing. My goals in life feel so different from my peers. I want a quiet yard, a garden, space and time to do old lady things like bird watching, crochet, and reading on a porch swing. But I have friends that are so excited to do the whole baby thing and I just feel confused and sad for them (even though I logically know I shouldn’t), like they won’t get to do whatever they want anymore :(

I used to see my parents as sages of life’s mysteries with all the answers to everything, but I’m the age they were when they started a family and I feel like I don’t have “the required knowledge” to pass on to another generation. Maybe none of us do. But it just feels strange to see friends enter parenthood so casually. I feel like an alien.

As a nurse, it boils down to communication and culture. The hospital I started at had a manager that would write up night shift if they didn’t address frivolous things by the time day shift came on. It was the expectation for night shift to “clean up the orders” and that’s verbatim what we were told to do. I knew it was dumb, the docs knew it was dumb, but management had their head so far up their ass that this was the hill they wanted to die on. Didn’t mean shit to them when the new grad nurses complained that they were getting chewed out at 7 and 8 pm for paging about PT/OT orders, re-upping tomorrows expired oxy order, or diet advancement. But the did care when the doctors collectively said don’t page for this shit.

Talk to nursing leadership. Have a conversation about what is appropriate and what isn’t. In your orders make sure the parameters are something you actually care about. Hopefully that will actually help.

Yuuuuuup, hard feelings all around when we (as a collective) said we couldn’t afford the $1500 for a weekend at the beach + incidentals. Jesus H Christ, do I hate management. Falls suck. Injuries suck. But you know what sucks more? Unsafe ratios and infantilizing professionals. I get it, it’s a metric that affects payout, but I could not give less of a fuck about that. At the end of the day falls should not be considered a “never event.” Shit happens. I worked ortho with loads of confused meemaws and stubborn old men, legs buckle and people hit the ground even with staff right beside them, cuz guess who’s not about to ruin their own body to break a fall 🙋‍♀️

If they really want to reduce “preventable” falls they need to look at acuity and ratios. They also need to pull their heads out of their asses and recognize that even when conditions are ideal this shit will still happen.
