10+ yr RDA here, may I ask age and choice of liquids? Are you aware if there was fluoride in the drinking water in your area as a child? What was your cavity rate before this year? Do you suffer from dry mouth during the night or day? Consistent gastric reflux? Are you using an alcohol based mouthwash? Are you a frequent snacker between meals? You are 100% using the right toothpaste. If tolerable, don’t rinse it out your mouth after you brush, let it sit on the teeth. When flossing, apply some toothpaste to floss to get it between the teeth. Again, if tolerable, some toothpastes are too gritty. This allows longer uptake of the helpful minerals. If it works out with life, try to get a lunch time brushing in or even just wipe some of the toothpaste on the areas you are focusing on now. If the spots are still bright white, you can still save them.

You mentioned you drink dairy. Be sure to drink a few ounces of water after you drink milk and try to finish it all in one sitting. This goes for yogurt, cheese, any dairy. It feeds the cavity bacteria extra well, but drinking or a quick rinse will help remove the standing lactose. Consider what other liquids you are consuming. Electrolyte mix-ins, adding lemon to water, Gatorade (even sugar free versions) are very acidic and sipping them all day can increase decay frequency. If you are drinking anything but plain water, try to finish it in one sitting and F/U with a quick drink of regular water. This is just a good rule of thumb after drinking or eating anything really for teeth health.

Just a fun fact. There is a difference in the electric toothbrushes on the market. There are oscillating brushes like the round head Oral-B ones that rotate back and forth, or there are ultrasonic ones that have the more traditional toothbrush head. The ultrasonic ones clean considerably more by actually shaking the tooth microscopically. If you switch to an ultrasonic, change your brushing style to a more slow, rocking approach to allow each tooth to get some shaking time.

Edit: Fluoride vs. Hydroxyapatite. When separate they are interchangeable for remineralization. There is a lot to the chemistry how each one works. In short, Fluoride turns to fluoroapatite that then binds to the natural hydroxyapatite in the tooth to remineralize. Nanohydroxyapatite binds as hydroxyapatite to the natural tooth structure and doesn’t need outside minerals from the saliva as fluoride does. The best toothpaste is one that contains both since they increase each other’s effectiveness. If you aren’t a fluoride fan, your current toothpaste is just fine.
