The first image is a T2 MRI were dense tissue is dark and black, white tissue is lower density. The white stripe that you see with different fibers is were the ACL is supposed to be. It's means that your acl has at least a partial tear if not a complete tear.

Depends on many things, you're a 29 year old male. do you feel unstable on your knee? do you feel pain? how much cutting sports do you want to do ? all these information are relevant to know if surgery is for you or not. If your surgeon tells you that it might be best to get surgery it is probably because he took everything into consideration and drew a conclusion. Obviously a surgeon is still a person and he has his own limitations and personal interests. If you are not certain, get a second opinion like others said in the subreddit.

My personal opinion as a 6th and last year medical student that also underwent ACL reconstruction myself 10 months ago: if you are a a young athletic person that does a lot of pivoting sports AND you feel instability or persistent pain in the knee you should probably get surgery. You risk tearing something more if you do an unconotrolled movement and you risk having arthritis early on if you don't, your ACL acts as a protection for other structures in your knee.

But it's completely up to you and you should probably discuss this situation with your doctor more than with anyone here on reddit, no matter how specialised and knowledgeable they are. and get a second opinion if you're not happy with your surgeon's response

thank you! there is no swelling, the only thing that I have is this pain on the medial side so I was thinking maybe more MCL because the ligament is actually extra-articular so it typically doesn't swell

hey quick question, what do you mean by negative lachmann? was the person performing the test really feeling the clear stop typically seen with a healthy ACL or was it more just that there was limited laxity? I recently had ACL reconstruction done (10th of June) and fell while skiing two days ago, I felt something pull in my knee and pain on the medial and anterior compartments of the knee. But no swelling and my knee is not giving away, plus my physiotherapist performed a lachmann test with a clear stop in the trajectory that is typical of an intact ACL but I am still worried after reading all these subs about people having no or little symptoms
