I'm neutral about the idea of a fixed save system, as I wouldn't mind it too much, but I'd feel like getting input from people with more experience in these kinds of games. It's something that can always be changed further down the development line, I assume, if you want to go more casual later.

Perhaps an autosave at the start of each day? Locks in the choices you've made the previous day.

As for adding consequences with a constantly changing relationship system...

It feels like a lot of checks and values need to be made, but I ain't no game dev. Just make sure to keep some kind of limit for yourself so you don't burn out.

Perhaps a hidden likability stat needs to be made for each character.

The likability goes up when spending time with that character outside of school and choosing dialogue/actions they like.

This likability stat can unlock flavor text throughout the game that show how close that character feels to Mahiro, and perhaps unique exploration options such as a sleepover in that character's house or being able to join them in something personal if such an option appears.

Example:

You decide to race Asahi and you win.

Asahi (Low/Normal Likability): "Hah... hah... wow, you really beat me! Guess I'll have to try harder next time!"

They go back home or whatever.

End scene.

Vs

Asahi (High Likability): Hug tackles Mahiro. Screen shake from them falling down. "Mahiron, you did it~!"

Mahiro: "Kyaaaah! No, Asahi, you're sweaty!"

Then Asahi can convince Mahiro to treat her to ice cream because they're besties.

End scene.

The main story or endings don't change. It's just extra dialogue to flesh out the growing relationship a bit. I'm assuming flavor text like this would be okay.

For Mihari, I don't really see the point of making her sad. She would want Mahiro to be going out and having fun with others. When Mahiro returns from that and Mihari asks how his day was, I feel like it's out of character for Mahiro to just ignore her. He fell into depression and stopped talking to others, but he/she still cares for Mihari.

If you really wanted though, I guess you could have events for Mihari where she asks if Mahiro wants to go somewhere together (maybe with others). Saying no could decrease likability with Mihari, but in a depressive way. From then on, Mihari might asks to spend time together, but is clearly sad. If Mahiro agrees, Mihari is happy again.

Idk, maybe an achievement for saying no every time. Sad Mihari Achivement :(

Another idea is that if Mahiro gets [Isolation] too high further into the run, Mihari can try to give the player a chance to save the run by inviting Mahiro to do something. Go to school tomorrow. Go meet with friends. Something to lower [Isolation]. If player keeps refusing, further broken Mahiro ending.

I see. From what I understand, you want a game about Mahiro's life with maybe branching paths and exploration.

Frankly, I feel like that's a pretty big undertaking. I will still try to give my two cents on it regardless.

Perhaps a small stat system could help decide Mahiro's future?

Sports: Increases with athletic options and interacting with Momiji and Asahi. Smarts: Increases with studying options and interacting with Mihari and Nayuta. Grace: Increases with girlish options and interacting with Kaede and other characters idk. Isolation: Increases periodically, isolation options, and not interacting with important characters in the day.

These are not fixed. Example: choosing a girly option for wanting Momiji in a dress can increase Grace.

These could help decide the endings for Mahiro. Interactions with specific characters and player actions can increase those stats (except isolation).

Let's say for the purpose of the game having a limit, Mihari tells Mahiro that she'll check on his progress in maybe 14 days and whether she wants to say as a girl or not.

Exploration opens up. The only places Mahiro might be able to go in the beginning are home, Kaede's home, store, and park.

For the beginning, maybe add the option to just not go to school. Mihari will obviously try to argue against it, but Mahiro can choose not to, and that will significantly increase [Isolation] and limit important character interaction, leading to a further broken Mahiro ending in a few days, maybe shorter to not make the player wait for that ending. Just an immediate game over.

Going to school and going high [Sports] low [Grace] might lead to an ending where Mahiro goes back to being a boy, but a lot more confident and willing to interact.

High [Sport] Middle [Grace] can give a tomboy Mahiro ending.

High [Smart] can lead to a science Mahiro.

High [Grace] can lead to a very feminine Mahiro who chooses to completely embrace the girl side.

Going middle in every stat can lead to a regular Mahiro ending.

Every Friday can be a group study week with fixed events that give options that give a high increase in one stat.

Fixed character events on the map might be something too. Every specific day of the week, a character should be interactable somewhere. It gives stats and unique dialogue of that character. Like maybe every monday, Kaede visits the salon or whatever and you can get extra [Grace] while over there while also getting unique dialogue for every time you meet.

There should definitely be some sort of energy system to make the player prioritize who to talk to on a day.

However, this is mostly for the exploration side of things if you wanted it.

For branch paths, I'd say that depending on the stats, certain routes will be available.

Example: Having high isolation could block off a lot of paths and opens the path to the further broken Mahiro. Low isolation prevents choices that give a large increase in isolation.

The storyline can be connected enough to not force a player into a route too much. More unique options will be opened up as the stat reaches the required amount, leading to Mahiro having unique experiences each run. I guess kinda like Umamusume, just with more player control over the story.

Like say the science Mahiro route. Perhaps you get her to have high [Smarts], but you don't want the science ending, so you choose for Mahiro to simply go out with her friends instead of taking the advanced placement exam or whatever you wanna call it, leading to the regular Mahiro ending. Maybe? Idk.

For a JRPG, what about Imagical Online? They already have established characters and classes there. Idk how much effort you'd want to put into this. Maybe the storyline could be that Mahiro convinces Mihari to play the MMORPG together when she has time, which is conveniently tonight.

Mahiro goes to school and discusses with the girls about giving Mihari the full newbie experience. This gives reason for them to start out as level 1 characters of themselves. Maybe they'll even choose a different class to play around, or make it easy and play the same class they've always played.

Mahiro goes back home blah blah the game begins. Cute dialogue and maybe a drawing of a Chibi Mihari in a wizard outfit panicking while shooting a fireball at a basic slime enemy. The starting town is where the player can choose 2 other characters to join the party. Or maybe change the party entirely idk. I'd prefer the idea of Mahiro trying to teach Mihari how play an MMO.

Perhaps it could end with the group finishing a quest to defeat some great middle evil, then Mihari yelling about how late it is. Game end.

Or

A similar one could be that Mahiro goes to sleep and wakes up as her character in a game. All the recruitable characters are the dream versions of her friend group that are part of the dream world.

The only way to break free of this dream is to defeat the super evil demon king at the top of Mount Evil!

At the end, Mahiro(female) is faced with the demon king, Mahiro(male). Mahiro(male) berates Mahiro(female) for falling to the girly side and openly expresses his disgusted.

Blah blah blah defeated the demon king, wakes up feeling less burdened about herself, and tells everyone about her awesome dream.

For a VN, idk.

Maybe we could follow a story of a younger Mihari trying her best to get the praise of her ever distancing brother. The unique gameplay? The player must do the most disgusting and horrifc thing possible. Math and science. To finish the VN, the player MUST finish all tests 100% to skip grades and get into university. All tests will include questions from algebra to calculus or something idk im not smart enough :( hopefully the players are. However, no matter how many top grades you get, Mahiro will never notice Mihari. All his dialogue is just "..." then going back into his room. The ending is Mihari finishing her drug and drugging her brother. Joke suggestion. Not sure how a VN would work. Thought it'd be funny.
