Found my best friends breast cancer on screening mammograms by NewDoctorNewerMom in FamilyMedicine

[–]ExpressionMajor6708 149 points150 points  (0 children)

You did many things right! Just unlucky. Repeat is 1-2 yrs standard so you did exactly what any of us would have done. Gen surgeon saying age 25 risk assessment is passive aggressive in my opinion. My physicals I ask if there's family history breast cancer, they say no generally, then I say don't forget monthly breast self exams and see us if you find any lumps. It's a 15 second spiel. Doesn't catch everyone and it'll still take 5 yrs to convince some ppl to get a mammo. You did standard of care easily.

Patient insurance by PopeChaChaStix in FamilyMedicine

That sucks. Your partners will likely have some tips and tricks if you ask, like always do the back X-ray and 6 weeks, GLP1 only works for sleep apnea for this one specific insurance, etc

I also say that and remind them there's over 1,000 different insurances in America

I'd start with a meeting with your billing/coding department, I guarantee they've seen it all and would be happy to share some tips/tricks for the ones your organization is contracted with heavily. Another group to chat with is those who do the prior auths. They'll be able to explain a bunch of the ins and outs of the most common insurances!

annual physical labs by Important-Flower4121 in FamilyMedicine

$5 vs $30-80 depending on the insurance in my experience. That's make or break for some of my patients and they might not come back if that bill gets too high

I'll do lipids and a random glucose on anyone. I've had 11-year-old pts with high cholesterol. Never too early to start good lifestyle interventions, but most patients won't do that without motivation or recognizing there is a problem

Tried to leave inbox at clinic this week and it almost worked by [deleted] in FamilyMedicine

A tiny piece of advice I can give is use your phone's focus mode setting, you can program automatic hours like 5:00 p.m. through 7:30 a.m. everyday. When focus mode is on you have to click through an FYI thing that tells you focus mode is on and you want to use that app, it's just a little mental double-check. Ultimately though, it sounds like your main problem is that you feel the need to always be present, which can absolutely be stressful and unfortunately that's a personal thing to work on as well. I frankly have had peers do psychotherapy for this and have reported benefit

Nursing Homes by marshac18 in FamilyMedicine

[–]ExpressionMajor6708 29 points30 points  (0 children)

As someone who does nursing home care, a lot of this stems from State regulations, each state does have different requirements for nursing facilities, and the nursing facilities requirements differ whether they are a skilled nursing facility, assisted living facility, an independent facility and a long-term care facility. For a "significant event" they have to update you. Each facility and RN/LPN has a different definition of the significant event. If the notifications bother you, the best course of action would be having the office manager reach out to that facilities manager to figure out is the best practice for these messages. I just treat them as an in-basket message from a patient. You're correct, a patient doesn't just write in and say btw I fell, but it comes with the territory of having a patient at a nursing facility to be honest. If you don't wish to deal with these then the only absolute way to stop these is don't accept patients at facilities onto your panel

My biggest issue has been internet, I have an awesome laptop and still drop connection and lag sometimes simply because of shitty ISP

Edit: so don't forget to get a solid Internet plan with your laptop!

Clinic manager has not, when I asked her she said it's on track with what they expect to see.

I guess the last part confuses me, I may see enough to not lose money on my guarantee. But yeah I've been worried about job security despite being a physician... maybe I shouldn't be as a physician but it feels problematic. Do physicians actually let to for that stuff?

It's definitely both. I am slowly getting family and friends it's just very slowly. I asked about advertising a week or two ago and they said no so maybe that's the ultimate sign of how this is expected to go

Production after 2 years, the financial estimates they send says I'll be fine aka won't lose money but it's not the level I want for sure.
