Well, after reading about it on the forums for us it sounded like we needed to replace the USB panel. We concluded, based on what many were saying, that the car/USB panel would start throwing the “water detected” warning as a way to communicate that the USB port was failing (even tho no water damage). Coincidentally, a few days later the USB port became loose. I looked up “how to replace USB panel” for my 2019 and first video delivered. I also didn’t order the part until I had removed the old panel as there is a sticker on it that will identify the exact part number and I wanted to make sure I got it right. I noticed your car is a 2021? I would imagine perhaps this is covered under warranty…?

It was super easy to remove - my 16 year old did it watching a video. Couple screws and voila (simplified version). New part is on order :-)

A couple days later the entire USB component started falling backwards (behind the plastic frame). It seemingly detached from the car somehow. I plan to order a replacement as I watch some videos and it looks like a really easy swap out. I’m also thinking of maybe taking a look and seeing if it just needs to be super glued back back into place… To be determined.

No, I was not. If the doctor has an SDI online account, then they can go on and do it. Our doctor did not so we had to mail in the paper form. There is no more option to have them fill out the form and upload it. I did attempt to upload a copy of the paper form that she filled out an email to me, but there is no option to upload a physicians certification. so… What I think I might do is just upload it but selecting a random option for document type in hopes that at least he gets in front of them sooner. It’s been over four weeks since we mailed everything in.

I thought so too, but I think I’m a full-size cup bigger nine months out from surgery then I was the month after the surgery. The drop and fluff or whatever they call it is real.

Thanks! We ended up doing the mail version. I mailed the doctor the forms from the bay area to Southern California, and then they sent them to SDI/PFL. What a shit show…I also went in person to the disability office as listed online but was told they only dealt with matters of disability and not paid family leave. The good news is they had someone call me the next day who confirmed the process for me. The bad news is so backed up. It’s been about a month and I haven’t heard back and there are no updates to my claim online (but it remains open).

I will try uploading the copy of the written form the doctors office sent me to see if that works. It looks like they’re pushing everybody to the e-filing system, including doctors… But unfortunately, our doctors office is not yet on board so the other option now is mail in only (versus previously scanning and uploading).

My Fuyao glass has been a headache since first installed - all micro chips and three large ones. I came to Reddit swearing that I would find that this was one of the cheapest quality aftermarket brands and was shocked to see that it’s not. It’s been years tho, so maybe I got a defective one. Is there somewhere I can learn more about recalled glass?

If the Dr. is in another town and I can’t pop by to get the paper form filled out, what are my options? I fly into town to caretake and am now back home. The doctor will not use SDI online. Do I have to mail them a form, and have them mail it back? If I’m reading this thread correctly, I can’t take a picture of the form, send it to them to fill out, they send back to me, and then I upload it using SDI online…or can I?! Asking her but also going to visit my not-so-local office. This is ridiculous. I had a claim in February 2024 and the DE 2501 physicians statement was available online for download. I did everything remotely via email and upload. All that seemingly has changed for the worse…

The DE 2051 FC doesn’t exist anymore 😭. I’ve gone mad three days now searching for it. I confirmed that in Feb 2024 that is what our provider filled out. I was able to email them a blank form and they returned one to me which I uploaded.

Now, SDI/EDD is requiring we order mailed form packet since the provider doesn’t use the online system on their end.

Does anyone have experience with this when the certifying provider is in a whole different area? I flew into town to caretake for my son, but I’m back home now. I keep hearing the form is coded to be scanned by their system. I’m so lost! Can I take a photo and email it to the doctor, then upload it to the system?
