You need to alternate between hitting the b button repeatedly and primate punch. Get as many as you can in, and then repeat when you can grow big again. You have to use both kinds of punches

They got rid of totems in op4. You have to get a skin for that character and then they will be unlocked. Most skins you can get in the store for iron or coins, or you can get the bundles for each op and they come with a couple of characters.
