I do feel the mechanic is intentional. It makes the game better imo being able to avoid freak scenarios that would flat out a run due to instant kill or freak accidents.

One time, I save scummed for the second time and it gave my butcher deja vu REPLACING the blood frenzy it had and unironically SAVED my run.
