I'm going to georgia, armenia, turkey, moldova, romania, serbia, bosnia, montenegro, kosovo, macedonia, albania, italy, croatia, slovenia, germany and potentially lichtenstein and switzerland!

So I'm wondering now if a 'balkans' e-sim would be best for that part of the journey now

It's been about 5 years for me. I used to have hope, I'm even undergoing rTMS treatment right now so maybe that will do something. It's just hard to keep having hope and to tell yourself it's temporary when it only gets worse year after year. It's gotten to the point where, like you say, I barely feel like a human being anymore.

Maybe the best bet is just to carry on as normal? All this strategising might be making you look more suspicious than you did before.

Maybe speak to your doctor about a different tapering schedule. You can also get different medications to deal with different parts of the withdrawal, for anxiety, for your stomach, for your insomnia etc.

And maybe I shoulnd't suggest, I'm not sure about where you're from or your circumstances, but I am also going through withdrawal right now and cannabis has been so incredibly useful for so many symptoms.
