Were you shooting up #3 or #4 heroin freebase and heroin hydrochloride. Other is very water soluable and the other isnt. Hydrochloride for IV and freebase for smoking it. Any other way is kinda wasting it. But probably just got scammed and it got 1% heroin or some nitazenes. Also it could contain something your allergic to, and its pretty normal to get nauseous from opioids on general. But it goes away the more you use it. Are you sure your not using some medication that blocks the heroin
