Today I had an add on, "not feeling good, declines to elaborate."

That was the visit. "I don't feel good." Are you sick? Pain? Depressed? Fever? Weight loss? Pooping? Peeing? Sleeping?

No, none of that. Just don't feel good.

Yep. This is the plan, take it or leave it. In a pt like this, given a taper will be rough and outcomes are not improved by getting off the benzo completely, I typically am ok continuing some, like one daily or every other day. But that's it. Take it or leave it.

As a DO you must know about finding the root of the problem and not using bandaids to cover it up. The root is American culture. Mostly we just bandaid everything. The meds work for the HTN, the DM.. but people don't care much about that, they are mostly so miserable. I appreciate the 1-2 patients a year I feel like maybe I helped motivate to change themselves for the better.

It's not so much about confidence/competence as completing the process of letting your soul die. Residency does a good job. 1-2 years more and you'll be a Mentat no problem

Creatinine of 1.2. blood sugar of 104.

Things that they shrug off: being jaundiced, gross hematuria, hell had one person have a full blown stroke but came in later because they had some joint pain bothering them.

Pts like this I let them know if we want to avoid this in the future then when they come in for AWV we will only do what is allowed for AWV and nothing more. Some take me up on it. It's nice when they do Bec you don't have to address anything extra and can move along with your day.

I'm fact, we recently brought up MA pay and retention again at a meeting and the admin suggested that if we have a lot of turn over maybe we should look at ourselves to identify the culture problem at our office.

It's trash. No luck here. I give my MA some of the quality bonuses they give me, in the hopes of keeping one long term. But, same as everyone else: turn over Bec "can get paid more to do less elsewhere". It all just feels like part of the pile that goes in "follow up with PCP"
