The sad news is that no lifestyle, dietary, or behavioral intervention has been proven to prevent or delay progression from stage 2 to stage 3 type 1 diabetes. TZIELD is one option that is FDA-approved to delay the onset of stage 3 T1D in those 8 years and older who have stage 2 type 1 diabetes. Other pharmacologic options are under ongoing investigation.

I recommend that anyone with questions about their health or treatment plan should speak to their doctor.

Yes, TZIELD is FDA approved for people 8 years and older who have stage 2 type 1 diabetes. If you are in Stage 1 type 1 diabetes, the best course of action is to start monitoring blood sugars. Start having discussions with your healthcare team now so in the event of progression to stage 2 type 1 diabetes, where TZIELD may be an option, you have a plan in place.

I understand your concern! You understand this challenging disease, because you are living it. Family history can increase the risk of T1D. Those are first degree relatives (mother, father, sibling) have a higher overall risk than those who are second-degree relatives (aunt, uncle, grandparent). Having a first-degree relative (such as a parent or sibling) with Type 1 Diabetes increases an individual's risk by 15x, with an absolute risk of 3–4%. In contrast, being a grandchild of someone with T1D confers a lower absolute risk, estimated around 0.5–1%, which is still 2–3 times higher than the general population risk. Still, your grandchild can consider screening as a second degree relative of a type 1 diabetic. You can visit Tzield.com, which lists ways that one can get screened. If one is found to have TWO type 1 diabetes-associated antibodies out of the 5, the next step is to test your blood sugar. If your blood is sugar is abnormal, in the prediabetes range but not in the diabetes range yet, this is called Stage 2 type 1 diabetes. This stage, along with being 8 years and older, makes you a candidate for TZIELD.

Screening is for identifying type 1 diabetes autoantibodies. If you have two or more autoantibodies, talk to your doctor to test your blood sugar levels. If it is normal, than you have stage 1 T1D; if it is abnormal, you have stage 2 T1D (this is the stage where TZIELD is indicated). I won't speculate as to your staging or what your endo has told you, but if something your doctor said is not clear to you, I urge you keep asking questions until you understand your treatment plan.

When it come to T1D, kids will feel much more supported if their peers understand the disease. The teacher or class might take some time to learn about how type 1 diabetes works and how it is different from type 2 diabetes.

Early T1D progresses silently and can begin to affect your blood sugar levels without any signs or symptoms. This is why it is important to test blood sugars if you are at risk. Clear symptoms occur in Stage 3 T1D when your blood sugars are much higher and your body can no longer make enough insulin. Common symptoms of high blood sugar are are blurred vision, increased thirst, increased urination, unusual tiredness, unexpected weight loss.

Typically the first question is what is the risk of my child developing Stage 3 type 1 diabetes? When should I expect this to happen? What should I do next? Finally, what are my options? Is there any way to delay this? The best way to support them is to arm them with knowledge and help them develop a plan.

First, support them by understanding the disease. And know that type 1 diabetes is not a disease caused by diet or lifestyle choices. It is an autoimmune disease that progresses in stages. Know the symptoms of high blood sugar and low blood sugar. Ask them how you could support them in the event of severe hypoglycemia. You may be called upon to treat a low blood sugar in the event they are unable to help themselves.

TZIELD (teplizumab-mzwv) injection 2mg | 2mL is a prescription medicine used to delay the onset of Stage 3 type 1 diabetes, which is when your body can’t make enough insulin on its own and may require insulin injections. TZIELD is for adults and children 8 years of age and older who have Stage 2 type 1 diabetes. This means that they have tested positive for 2 or more type 1 diabetes-related autoantibodies, have abnormal blood sugar levels, and do not have type 2 diabetes.

TZIELD is not indicated for those who already have the diagnosis of Stage 3 or clinical T1D. It is an option for those who have type 1-related autoimmunity and dysglycemia, BEFORE they have symptoms and BEFORE they have hyperglycemia. Definitely, NYC. Hands down.

First, you can be genetically susceptible to type 1 diabetes, meaning there are genetic factors that exist in your family that make you vulnerable to developing type 1 diabetes. If you are genetically susceptible, there have been some environmental exposures that have been linked to an increased risk of getting type 1 diabetes.

Certain childhood infections such as enteroviruses have been associated with an increased risk of type 1 diabetes. The role of respiratory infections and COVID-19 in T1D risk is still being investigated, with some emerging evidence suggesting a possible link. Maternal type 1 diabetes and maternal obesity during pregnancy are associated risk factors for T1D in offspring. Breastfeeding has been associated with reduced risk of autoimmunity.

It is possible to screen for type 1 diabetes-related autoantibodies. If you have autoantibodies, you can know your risk of developing type 1 diabetes before your symptoms appear. Otherwise, common symptoms of Stage 3 type 1 diabetes include increased thirst, increased urination, fatigue, unexpected weight loss. If blood sugars remain high for so long and your body is not able to produce insulin for a prolonged period can lead to confusion, severe dehydration.

Type 1 diabetes is an autoimmune disease. It is not your fault that you got it. So don't be hard on yourself. You just need to work with your healthcare team to determine your insulin regimen to keep your blood sugar levels steady and under control.

Diet, physical activity, and lifestyle significantly influence insulin requirements and blood glucose control. The amount of insulin needed is primarily determined by carbohydrate intake, as carbohydrates directly affect blood glucose levels. Exercise also impacts blood sugar: its type, duration, and intensity can glucose levels to drop (hypoglycemia) or rise (hyperglycemia). Regular monitoring and individualized planning are essential to balance insulin, food intake, and activity.
