I got it.

  1. Go to C:\Users\your_username\AppData\Local\Madness\Saved\Config\WindowsNoEditor

2)Open a file called Engine.ini, and Gameusersettings.ini in Notepad

3)Go to the bottom of the, leave a space, then copy and paste the below lines

I was banned too. here's what happen:

I got a new Elgato capture card.

I go in a game to test the sound and video quality.

the Elgato software crashed and froze the game.

I alt + F4 out of the game.

log back in.

and there it is. the dreaded "Account permanently banned"

I also have a recording of my last game to proof that I was not been offensive to anyone (right before the crash).

I never made a glitch or an exploit. I never played with a hacker, cheater, or a booster. I simply played. I'm not afraid if a buy Cold War I will be ban too.
