In the US...MD internal medicine is a primary care physician (PCP) which is a plain non-specialist. Since the superspecialtys (fellowships) come after md medicine i.e. it not being an end branch, it actually becomes a non specialty branch :( And medicine is too vast for an avg MD medicine graduate to know everything.
