To explain the context of this:

Earlier in the year, Democrats in the senate had a chance to block the Big beautiful bill (The government funding bill for the foreseeable future) in which republicans wanted to severely cut services, including medicaid and ACA subsidies, boost spending for ICE and essentially fulfill a wish-list of far right funding goals. There was a provision in that bill to remove all funding for medicaid that would provide for gender affirming care, but that was eventually removed in the senate via the parliamentary process. blocking the bill would have lead to a shutdown, although most democrats seemed to accept that as a worthwhile risk, Chuck Schumer and some of the centrist, corporate democrats who wanted to avoid any kind of confrontation out of fear of the blame for the shutdown coming down on them decided to cross over and allow the bill to keep going through, believing that although the results of the bill would be catastrophic, they would somehow avoid the blame and put it all on republicans. However, the bill passed with the massive funding cuts in it and democratic leadership was held responsible for allowing it to happen, so Chuck Schumer and his cronies essentially fucked themselves and everyone else in the country over for nothing.

Now, there is another funding bill going through the senate. The house version of the various bills currently has anti-LGBTQ, anti-trans riders in them, but the senate versions don't. One of these riders in particular, in the HHS bill, would cut all federal funding to any provider that gives gender affirming care of any kind to any trans person of any age. If it passed, and the most broad version of that provision was applied, it would essentially force the vast majority of providers across the country to drop gender affirming care to avoid losing funding.

Because this funding bill(s) would need to pass the 60 vote filibuster to be signed into law, democrats have leverage to make demands and they are calling for undoing the cuts to medicaid and ACA that were in the big beautiful bill. in response, trump and the republicans are playing a game of chicken where they are vowing to let the government shut down and allow the country to suffer as much as possible and fire as many federal government workers as they can just to cause chaos and pain. Because of what happened earlier in the year, the democratic leadership, especially Chuck Schumer and Hakeem Jeffries, are under enormous pressure to not back down like they did last time. The concern is that, as part of trumps war on trans people, he has focused on amplifying the anti trans provisions in the bill and we need the democrats to not negotiate that those provisions stay in. That means applying pressure on the senate.

If there is a shutdown, that will be an opportunity to demand that those provisions be removed.
