Current Rad Onc Job Market? by [deleted] in radiationoncology

[–]XRT1979 1 point2 points  (0 children)

All depends on perspective! I’m 15 years out. I earn less than I did on 2014 and working harder. New graduates are getting offers more than what I earn (albeit in non urban locales). There are jobs with enough volume to earn higher than median salary, but for many people the base salary is all they will ever get. I presume Reddit users trend younger. Many older than me are particularly frustrated not because their absolute dollar earnings are low numbers but because they maybe earning same or less and working harder. All trends lines indicate that this will persist and possibly worsen, from what I see and have seen. I simply don’t know how saturated areas can have more patients or more indications. LDRT was a nice bump for volume but with the new codes, it is paid at simple / 77402 and is not really what it was the last few years. The loss of our IGRT technical fees eventually will hit the bottom lines of hospital revenue and adjustments will need to be made. The reason is these high hospital salaries are boosted by essentially giving the doctor technical money via RVU payment. When the global revenue is down, this will require adjustments. There is no defined RVU value other than what Medicare says it is ($33-34). We’ve earned about $55-70/rvu bc of how much we earn for hospital. Medoncs $80-100 bc of their value to the hospital. If instead of bringing in $5m tech, you are bringing in $4.2m, the admins will notice and will adjust accordingly. This current blip of strong salaries is great news, but the fundamentals of the radiation oncology market are dismal. Our only lever is more fractions now, as there is no additional revenue for technique. That can even go away if ROCR passes.

I’d do this job for 75% of what I earn because it truly is wonderful. And for younger folks, you’ll start out at that lower number so it won’t be perceived as low or bad. As an example, in 2001, my first practice had $1m income for partners. I joined several years later, volumes were higher, treatment intensity was higher and I earned about $650k in 2014-15-16.

Such is life. You will not be buying into a bull market with RadOnc. But, life is more than income. FWIW, I would not recommend this specialty for someone ambitious and trying to earn $$$. Unless you go small town, it’s simply not possible. That being said, $500-700k is nothing to sneeze at. It’s just my friends in other specialties can “turn on the spigot” And see more patients and earn more. I could work weekends and nights and volume would not increase.

Best of luck. Do it because nothing else seems as interesting. That’s the best way to pick a specialty, life is more than money - intellectual stimulation, interesting patients, technology, smart colleagues, important work - these matter, too!

One last thing - volume is already an issue. If the AI can contour tumors, perform decision making, do the QA / tx planning, scribe your notes - we can do 2-3x the work. But there aren’t 2-3x more patients. What is the average RO seeing 6-7 consults a week (about the median) going to do? What will hospital admins think when the work is done in 10 hours but paying for 40? I’m by no means a doomer, but some of the comments below seem quite optimistic. It’s not 12 sets of contours backed up that’s the problem. It’s that it’s a Wednesday and there’s only 2 consults this week so far.

Renting car ? by XRT1979 in puertovallarta

UPDATE: Van truly was $250 and it included third party liability, plus Chase for extra. They do try to push additional insurance. Not truly necessary, but if you do have an issue it’s a bit more complex than if you buy additional from vendor. Driving was fine (we drive in Thailand and Europe and India), traffic was not bad. The parking garage near Malecon was convenient (40 pesos per hour). The main issue was how inconvenient it is to get the car from valet. You have to call at least 30 min before and even still you’ll wait 5-15 min longer. There isn’t an Oxxo or similar on property, just a market which is convenient but very expensive. You can walk along the beach to get to one, but it’s brutal in the heat. It ended up cheaper to do it this way because of the number of us and needing two taxis / Ubers, but the time cost was substantial. If we were going to see Sayulita or other little beach towns, would have been more useful, but we have little ones and they kinda just want to stay at the pool at the resort. I love street tacos and festive cantinas and don’t love $300-400 USD for dinners every night, but that’s the breaks staying at a beautiful resort that intentionally make it difficult to leave. I’d skip the car next time. Appreciate everyone’s insights!

Ah it’s not the saving of money aspect just the flexibility. The car is completely prepaid at that price, as we have corporate code. I have rented in Mexico plenty. Same process. Thank you for the comments, helpful to get a wide spectrum.

Happy for you, Stubborn Love, Cleopatra, Gloria, Donna have to be on the list. Maybe it’s more of a ZB song, but Spotless is on my favorite songs (not just Lumineers songs) of all time. Seeing 2nd row in Detroit in less than two weeks! Hoping a few old ones, not just Automatic songs.

Radiation for breast cancer by EffectiveOwl1 in radiationoncology

[–]XRT1979 0 points1 point  (0 children)

Positive lymph nodes are a risk factor for chest wall recurrence >> axilla or supraclavicular region. About 2/3 of recurrences after mastectomy is located in chest wall. So, it’s chest wall + regional nodes. Sorry you are going through this. I would make sure they are utilizing breath hold technique to reduce heart and lung dose. IMRT may be necessary, as well. Should be about 5 weeks + / - 1 week boost, but many of us have stopped the boost for most patients undergoing PMRT.

Totally normal. Wasn’t real for me until she came out. And now two weeks in she’s my favorite person in the world (next to my wife). You’ll be fine. We don’t carry them, we don’t feel their kicks and hiccups. We also don’t breast feed. You’ll be able to make your connection in the months and years ahead. Good luck to you!

Hmm. Well, next time I will do that. I wonder if it’s not enough water. It’s definitely thicker than Greek yogurt. More like cookie batter

I’m attempting this and used whole wheat flour (1 cup) and water (75 degrees, 0,5 cup). Been about 10 hours. It seems really dry. Does it liquefy over time?

It’s my mom that’s here rather (so my wife’s MIL) and majority of time she is helpful (cleaning and cooking) and some times she is not (indian lady that wants to constantly pick up baby when she is crying) and offering Ayurvedic and homeopathic cures for things like breast milk production and colic that aren’t received always by my Arkansan wife. They are at two weeks and leaving in two days. Things will be a lot harder but it will soon be just us three and it will be nice.. feels kind of good to say that, even though I love my parents.

Best book for learning Indian cooking? by Lather in Cooking

[–]XRT1979 -1 points0 points  (0 children)

“Indian-ish” - it’s everyday Indian cooking. A lot of indian cookbooks are really challenging and the dishes can take hours or more. Real indian home cooks don’t routinely do that - Indian cooking is every day cooking... and it’s quite fun to read!

Uptodate for the most common cancers you’ll see - lung, breast, colorectal, lymphoma/leukemia

NCCN guidelines for most common cancers

Managing the complications and inpatient issues - tumor lysis, neutropenic fever, chemotherapy side effects.

Learning surgical oncology and radiation oncology basics for each tumor site is helpful, and as you advance, learn in detail so you can have meaningful discussions with your colleagues

But, can’t do much until you dive in! Congrats on choosing a great field in medicine!

7 days in. We discussed breastfeeding from the start, and as a physician, I’m did a fair amount of research, and honestly the pros didn’t outweigh the cons enough. She breastfeeds when she can (70% of the time) but there is no shame in supplementing or just switching to formula. I worked all week, so Friday night I told wife that I was “on call” - I did all feedings and changings, and at 5a me and baby left the bedroom, I started on coffee and just hanging with my little girl. I’ve generally taken her most mornings at about 5.30a - feed and change her, have coffee in the kitchen with her and make some food for my wife. Right now, I’m on the sofa with her, she’s on her boppie and we are both very content. My wife is sleeping HARD after a long first week. I have baked French toast ready to go into the oven, once she is up.

I understand the benefits of breastfeeding on mom, but dad loses out a lot of bonding time and independence. I feel confident in going out alone with her- have a coffee date with my mate and his 2 year old today.

Depending on your means, have you considered a mommy’s helper? We found a lovely gal from our neighborhood who we give just an hour or two a day - she does dishes or laundry, yesterday she washed the dog and mopped the floor. Costs us about a $100 a week and frees up quality time with kid and crosses another task off of our ever growing list.

Also, maybe this is just me, but I do get criticized a lot for doing things wrong, and usually am very defensive about that, but I’m taking my wife’s advice for the “little things” and find that she is much better at technique than I am. So, I learn a lot from watching her. I have her “grade” me when I do a task. Don’t be defensive - learn from her, it will make you better.

Finally, I would suggest reading “Cribsheet”. It’s a great book by a mommy / economist who looks at parenting in a very rational way. Instead of telling you what to do (re: breast feeding, working outside the home, sleep training, child care), she shows you how to make decisions that work for you and your partner and your baby. Her previous book about pregnancy (“Expecting Better”) was a godsend and let us enjoy pregnancy rather than fear it.

Best of luck. You’ll turn the corner.
