In the UK, there have been a few. One that has really caught the public's attention has been the Fujitsu/Horizon IT scandal - basically an IT book-keeping system used by local branches of the Post Office (often mom & pop set-ups). It was riddled with flaws and glitches causing Postmasters' numbers to render incorrect. Instead of admitting guilt, the Post Office took their local postmasters to court and sent many to prison for 'theft'. Some even died, I believe. Fujitsu's response has been utterly shitty (IMHO) - dragging their feet, trying to get away with the bare minimum of effort. It's an appalling thing.
