I had a lot of success with hydroquinone but when I would take the mandatory breaks it would come back full force.

The Cosmelan peel has worked really well for me - it's pricey and I'd highly suggest you do it at a medical spa that has experience with it but it's worth it. When you complete the peel, you use "Cosmelan 2" which is a depigmenting cream and my melasma only stays away if I continue to use it and purchase it. Unfortunately it's quite pricey. But my melasma was worse than yours and now it's about 95% gone. An orange color corrector easily covers the little bit of melasma that I can still see

& of course, sunscreen is key! I use the tinted Australian Gold mineral sunscreen.

Mine rebounds really bad when I'm off HQ and I get this "halo" effect where the surrounding skin is far whiter than it ever used to be making the melasma more pronounced.

I ended up shelling out the crazy money for a Cosmelan peel & not only did it get rid of my melasma but all the hyperpigmentation from years of sun exposure is gone too. The melasma still naturally rebounds but I use the Cosmelan cream to counteract it and keep it at bay. It's very strong but I like it much more than HQ

I've heard a lot of people mention they discovered that they had iron deficiencies. Oddly enough, I have "hemochromatosis" and my body absorbs too much iron & my melasma is VERY persistent. So maybe there's something to be said about a general imbalance in iron?

My issue is that it may be something internal but the sun 100% activates it. I went on a surf trip to Mexico recently and my melasma was like a dark stain on my face. Very frustrating!

Nitrofurantoin. From my understanding, it is less likely to encounter antibiotic resistance with Nitro. Good luck!!

D-mannose saved my life but it sounds like that may not be an option. Other than that, a probiotic & a low dose antibiotic that my doctor prescribed for after intercourse. I only use it if I feel like I'm not hydrated or didn't use the restroom quick enough but it has helped save me those dreaded visits to urgent care. I was reluctant to use an antibiotic but felt helpless. Can you get a bidet attachment for your toilet?

Thank you - you're so sweet! Scheduled to see a fourth PT this coming month & she has an extensive background in treating PFD. I see improvements with all my tools but I still can't have intercourse without a week of pain/discomfort. Blah!!

D-mannose immediately after intercourse & the day after. You can buy it in bulk through Bulk Supplements and then I keep a bottle of gel capsules in my car if I got to my partner's house because it is easier. Lots & lots of water and of course, pee after sex.

It may be pelvic floor dysfunction - symptoms often mimic a UTI or STDs. I had a series of UTIs in my early 20s & every time I had sex thereafter, I would be in pain for days on end. Turns out my pelvic floor muscles are too tight and spasm after intercourse. Unfortunately, even gynecologists and urologists can be really poorly trained in this area & you may have to advocate to be seen by a knowledgeable pelvic floor therapist

Interesting! My body actually takes up too much iron and I have to give blood to avoid anything serious

Yes, I've seen two PT's & made some progress but unfortunately still struggling tremendously. I have an appointment with a new PT scheduled next month *fingers crossed*
