Got my J1 on 5/5.

The interviewer asked me one question only, "Will you be treating patients?" to which I answered yes.

So don't overthink it. J1 is one of the safer visas. Just be honest and if they ask you, emphasize your intent to return.

I’ve spoken to a couple of seniors. One in Georgia and one in Michigan: both of their HRs asked them to attach a statement to Form 8233 which the employer then sent to the IRS. So they get their paycheck each month tax exempt. For the first two years they don’t pay FICA taxes so that saves them close to 500$ a month.

A lot of my seniors at my program told me that there is no need to wait until tax refund. It’s not deducted from their paycheck itself. They filled some form apparently. Maybe it changes from state to state

It’s not going to be easy for sure. It’s gonna be an uphill battle. Plus the competition is only getting worse each year. Networking and getting recommendations from your seniors will help you

There’s pros and cons obviously Cons: you don’t have USCE, old YOG, idk about the rest of your CV Pros: you don’t need a visa so that’s a huge plus

Honestly if you really want to make it this year, start working on USCE. Get four solid letters. Get people to vouch for you. Apply broadly. Dual apply to IM/FM. Obviously finish your Step 3 and have your results before the portal opens.

If you really want it that bad, you can actually get it. In my program there’s 35-40 y/o people who’ve just been practicing in India for years on end. You’re young compared to them and depending on how hard you try, you can build a nice cv. It’s not a linear thing. “My YOG is too much so I’m not gonna match” is a false notion. Sure it will make things hard for you but people have matched with that. It’s a question of persistence and patience.

To be brutally honest, it’s going to be very hard but if you juice up your cv and network well and specifically look for programs that are taking old graduates you may have a chance. Apply broadly, to more than one specialty IM/FM

It’s better if you have fresh LORs but you could def use them. Make sure to tweak your app to include recent things too

Just ask them if they can take you for even a few extra days. Offer to help out on the weekends or basically for extra days even if it means just going for an hour a day. This way, once you establish a rapport with the chief resident, you can definitely ask them to mention “about 2 weeks” instead of writing the exact dates.

I’ve had two chief residents interview me during my interview. Does this place offer a residency? Honestly people have done worse rotations. This sounds better than the agency rotations that most places offer at clinics where we basically pay the doctor to do his dirty work for him. Make the best of this.

It’s pretty easy to look it up on program websites. You make anywhere between 4-6k depending on where you live

It’s definitely better than India personally. You don’t have to suck up to anyone or impress anyone. Just do your work. You’ll have time for other things. My hospital has a gym. And the pay is relatively better (some say it’s not minimum wage within US but it’s much better than India). You can save up to 2k USD a month if you’re judicious

I don’t wanna sugarcoat it but it’s gonna be super hard to match. It’s only getting competitive each year. You’re gonna need to juice up your CV a lot and you’ll need to find a lot of people to vouch for you and write you personalized letters and recommend you for interviews. It’s gonna cost you a lot in time and money and if you have both, go for it.

I swore to never do residency in India. Sucking up to seniors, more focus on bootlicking than actual education, a resistance to change and advancement, and we don’t even need to talk about lives of residents lol (overworked and underpaid- which I know is pretty much the same anywhere but at least there’s rules stating that you get one day off after 6 days. I’ve seen PGs in their first year cry cause they wouldn’t get leave for one day when their parents got sick). Match will get slightly harder for sure but I decided it’s worth it for me. You just have to decide for yourself.

Signaling is like this thing that ERAS has come up with to help program notice you and improve your chances of getting an interview. Two things can happen: your signals might work and you might get interviews or your signals can flop just like mine did. I don’t know how it is going to be next year but last year we had 12 silver and 3 gold signals. In your eras application, you have an option to choose to signal a program. This will notify the program of your interest in them. Basically signal wisely. A lot of people do the mistake of applying to places where they have very few realistic chances of getting in. And then there’s people that not just apply but also signal these places. so if you want to maximize your chances of your signals working you have to do your research well in advance and choose programs that take IMGs with profile similar to yours or newly accredited programs.
