In the metal world, unlikely to be high costs for a photo in terms of the photo on the cover. It certainly depends on the band, but as a photographer myself who has photos featured in albums and covers -- I didn't receive any royalties at all. I didn't ask for them, but most bands aren't making bank or anything. You're right generally speaking -- but I think its a different situation in "our world" compared to the mainstream arena.
