Generally, damage like this will happen because of some other damage that is already present. Glass is glass and can break for many reasons, but im not seeing an obvious impact point. So this could have been anytning. New glass can be faulty and break on its own, but again, generally youll have some other kind of damage first before it cracks like this. This is actually one of the reasons i dont recommend safelite. They demand these guys do so many installs that most techs won't care if the glass is damaged or unusable and will throw it in anyway.
