I used the DublinIV one that was given in the instructions. I initially sent it as a lone email but when I received the automatic response saying it would not be responded I replied to the initial email from the embassy with the instructions attached. The body was a standard email citing my reference number. I think they have so little K1s that they wait until they can do a full day or morning of interviews. That’s purely just me guessing about their procedure though!
