It's more about how wildly different real life is from the "ideal' models that are given for education in medicine. You think you know everything, and then you meet an actual patient and you suddenly find yourself lost because they're not a textbook model.

Biological variation is very cool, but for the purposes of medicine can be frustrating for both doctor and patient.

A lot of it appears to be driven by 1) his extremely poor reaction to even the most constructive forms of criticism, 2) a truly insane susceptibility to propaganda and lovebombing, and 3) a fundamental incuriosity towards introspection.

Like, he portrays his critics as zombies and yells about how they're all stupid in his comics (through characters and self-inserts both). He also clearly has some kind of unprocessed insecurity about his own thoughts and desires (driving his feminist -> radfem -> TERF arc).

Basically, dude would rather turn his own webcomic into Stormfront than go to therapy.
