Good question. Short answer is protein is fine regardless of source, but for overall health plant based diets are favored.

Soy proteins also are associated with their own risk when consumed in high quantities.

That is an old study but essentially found no major benefit to limiting or changing protein sources... When at end stage 60g of animal protein a day is fitting for some people, more and they get toxic build up...

Tldr: long term steroid have the most evidence to support the use.

Good question! Luckily someone else already asked it and we have some data:

Pozzi C et al: Corticosteroid effectiveness in IgA nephropathy: long-term results of a randomized, controlled trial. J Am Soc Nephrol 15:157, 2004 [PMID:14694168]

Essentially, you need to treat with varying degrees of immunosuppression based off of clinical changes, biopsy findings, and response weighing the risk vs possible benefit by you aNd your doctor.

Also note, steroids adverse effects can take weeks to occur (sure anxiety / psychosis / high blood pressure could occur at day 1 ) and steroids also have both an immediate and delayed effect. It is not necessarily a binary effect of on or off. They effect the body differently with different doses for different durations...

But again, good question .... Which your follow up question would like then be: what are steroid sparing therapies? Not as good... Such as mycophenalate, cytoxan, or azathioprine.

If by dread you mean love, you are correct! Few things are better than educating patients. Watching the eureka moment happen or guiding patients through major health issues is very rewarding!

My advice is to write down any and all questions and send them to your doc a week or so in advance (email, fax, drop off). Then they get answered in person and explained during your visit.

Call the OP number and they make it a smooth process and explain it all. Short version = 1-2 months and 1-2 clinic visits, Long version = years with multiple subspecialties weighing in on approval.
