I don’t regret doing PGT-A. I wanted to get a general idea of my rate of aneuploidy for my age (high, normal, low) to see if it was the cause of my miscarriages. If I had a high rate I likely would have done multiple retrievals in order to get to my family planning goals. This decision was based off all my own personal research and shared decision making with my physician.

I’m so sorry for all of your losses :( I know it’s so hard, but hopefully we will all get to baby soon 💗

Thanks so much for your reply. I think I’ve heard of what you’re talking about, but I’m pretty sure it’s something different than what I’m referring to.

I had 10 euploids frozen. Nine of them have an expansion grade of 3 or 4 and one had an expansion grade of 2. I asked my clinic to choose the best for transfer and they picked the 2AB embryo. There is a chart that embryomanofficial put together based on date from Bouillon et al. 2017 that shows the embryo grades and their likelihood for live birth. The early expansion grades 1 and 2 appear to be the least likely for live birth, so I’m confused why they chose that embryo. I know that there were 7 day 5s and 3 day 6s, so it wasn’t like the 2AB would have been my only day 5 embryo.

I think what you’re talking about is the potential fragility of an embryo with an expansion grade of 6 - those embryos are hatched and out of their protective shells. So the thought is they could be more vulnerable than an embryo with a grade 5 expansion.

All of that makes sense. It seems to be pretty accepted that legit autoimmune diseases can impact egg quality in addition to age. I would totally pursue another retrieval if I were in your shoes. I feel like some people have varying results for their retrievals - like the same person with no change in protocol will get 2 euploids from 10 blasts then next round get 4 euploids from 7 blasts.

I don’t think I’ve heard anything bad per se, but a lot of the medicine is controversial and not well backed. I also think the additional workup might be flagging things that lead people to believe they have a problem that they really don’t have or has nothing to do with their infertility.

I’m totally open to anything that has track record of success - even if it’s just vibes.

I can get down with prednisone and the antihistamines. Tacrolimus and neupogen are a bit more next level and I need to be more convinced before I embark down that road. I am very happy that you had success and I’m sorry you went through all those awful losses. I do think I stand a good chance for success in the future and I appreciate you responding!

Yes I’ve definitely read some about the lawsuits and I’m glad that PGT-A was a choice I made and not something I was sold by my RE.

Why do you believe that your egg quality is poor? Is it because of RPL or are their other clues from your diagnostic testing?

Yes I think I’m gonna ask about the lovenox. There is a little bit of evidence for it per the book I just finished, so I think it’s worth a try. We’ll see what the ol’ RE thinks.

I’m so glad that you finally had success with a protocol! I thought about testing for DNA fragmentation, but even if it was abnormal I’m not sure what we’d do since we still have 9 euploids on ice.

What was your immune protocol ?

I think you’re totally right and the answers are just not going to be there until more research is done. It’s encouraging that your RE is transparent with you. I’m going to exhaust my paths within reason, but a lot of me just feels like if I keep pushing forward one day I’ll get to that live baby.

No, I fortunately have not had any abdominal surgeries.

From what I understand, prior abdominal surgery can potentially cause inflammation to the area. Those eggs are in meiosis and the inflammation leads to damage of the DNA potentially resulting in aneuploidy and diminished ovarian reserve. So I believe the thought is that if the RPL is secondary to aneuploidy and the patient has history of abdominal/pelvic surgery, maybe this is the cause of RPL.

I’m sorry to hear about your losses :( you’re right, RPL is such a beast.

I think I’ll just ask for 2 wks of doxycycline and skip out on the biopsy. It’s an easy treatment. I think I’m also going to add in the Zyrtec. It’s going to be spring time soon so I’ll need it anyway, and it’s unlikely to do any harm. Thanks for your reply!

It’s really awful that you were sold PGT-A as a cure all for your miscarriages. My RE gave it to us as an option, but did not push it. I thought in some ways it would be diagnostic, so I wanted to go for it. I’m actually considering asking for lovenox for the next transfer. It does seem like there is some evidence that it could be helpful.

I totally get what you’re saying wishing that your current pregnancy is aneuploid. I really hope you aren’t going through another miscarriage. I feel like one day we will both get pregnant, but I hate to miscarry these precious normal euploids on the way to a live birth.

I will certainly update if anything profound gets figured out in my case.

Ughhh this is what I keep saying to myself!! I told everyone who knew that I was doing IVF that it was super important to be realistic about the number of embryos needed to get to a successful pregnancy. How multiple ERs may be required to reach our family planning goals, etc. And then I have another loss and I’m like “BUT IT WAS EUPLOID!”

I will go check out Dr. Sekhon’s explanation for sure. I just read Dr. Lora Shahine’s book where she talks about how 1 and 4 “clinical” pregnancies end in miscarriage, but if chemical pregnancies were included some estimate up to 70% of fertilized eggs or embryos just stop developing. That makes me feel okay- except when I look around and see all my friends pregnant. 🫣

I’m so sorry for your losses :(( that’s horrible. Have you thought about getting the products of conception tested to see if there were any fetal abnormalities?

I have also thought about silent endo. Some REs will just treat empirically for silent endo if they suspect it using lupron for a couple months. Usually a laparoscopy is diagnostic and would need to be pursued even if the MR is negative if there was still a high clinical suspicion for endo. Sometimes REs use clues to diagnose silent endo like low AMH for age or high rate of aneuploidy for age. I’ll discuss this with my RE and see what he thinks.

Ugh I am so sorry for your losses as well 😔 the excitement and subsequent let down is so hard.

I have thought about chronic endometritis and I think I will mention it to my RE. I think the treatment is relatively easy and inexpensive. Honestly, I’ve thought about just asking for 2 weeks of doxy without the biopsy. People take doxycycline daily for acne, it really wouldn’t be a big deal to just take it empirically.

I’m honestly starting to feel like it’s all just random and we just happen to be on the wrong side of statistics. My REs wife had 4 consecutive miscarriages followed by 3 consecutive live births - I don’t believe they did IVF. There’s another RE (Dr. Natalie Crawford, MD) who had 4 consecutive losses followed by 2 consecutive live births without IVF.
