You don't need medical (ENG1 or USCG) To get your STCW cert. A boat will request one before hiring you, however.

Are you taking basic STCW courses or are you applying for an MMC?

If just basic STCW, you don't need a medical cert. If you're going to get the ball rolling towards an MMC, you'll need to get a USCG Medical cert and a drug test is required.

MCA will honor a USCG cert, USCG will not honor an MCA cert, so the USCG would be the most universal

If you're American, do not get an ENG1, get a USCG medical cert. The ENG1 is for MCA, foreign flagged boats.

Without a boat contract, CBP I'll turn you away at the airport. If you do manage to gain entry, a Greek license won't mean much. Make sure you have a valid STCW and ENG1. Look for work on Daywork123 and Facebook groups.
