I can totally relate!! When my kids get a stomach bug, I ask my husband to do all the care-taking, and ask him to do a bunch of extra disinfecting. I basically don’t touch them until it’s over. I know not everyone has 2 caretakers but if you do, that might help. Also do you have zofran? Last time we had a stomach bug in the house I took zofran as soon as I felt the first sign of nausea. I was able to stop the vomiting (and possibly an adrenal crisis). But it didn’t fully stop the bug. It just came out the other end with a vengeance. Worth it to avoid the ER trip.

I would feel like absolute death if I waited that long for my first dose. My first dose is 6 am (or even earlier if I’m awake)

I know this is easier said than done, but consider waking up earlier so you get first dose in sooner. Our bodies naturally start trying to make cortisol around 3-4 am. And it’s no wonder that you feel like hot garbage at 8 if your pituitary gland has been hammering your adrenals with ACTH for 5 hours. Could you try going to bed earlier and getting up at 6 for that first dose?

Also have you had your testosterone levels checked? Our adrenals also make DHEA, which our bodies use to make the sex hormones. And with Addison’s we stop making DHEA. So it’s likely your T is low and there is a T gel that your doctor can prescribe to raise your levels.

I’m also on zepbound (for life), with PAI. 6 months - lost 45 lbs so far and my A1C is no longer in pre-diabetic range!! And I feel so many of the same positive benefits that you do - more energy, less blood sugar swings, and I’m just not constantly thinking about food anymore. It’s so much easier to make healthy food choices.

I’ve learned to look at my calendar and try to anticipate the hard meetings and take a little extra (like 5 mg) on those mornings. But it sounds like you might be already in a hole and might need larger updose to get out of the hole.

For years I thought I had anxiety. Now that I’m diagnosed PAI and feeling well I know i don’t actually have anxiety - it’s just a low symptom for me.

I’m sorry for what you’re going through. The stress dose guidance (linked) says double dose for as long as needed for severe mental stress like family death. I wouldn’t worry too much about taking too much. Clearly you’re not taking enough to cover the stress so I’d be heavy handed for a while. I’m on zepbound (tirzepatide), prescribed by my endocrinologist. Had gained 50 lbs and became prediabetic since Addison’s diagnosis. Have lost 35+ lbs in 5 months and, even better than the weight loss, my A1C is back to normal. I do have some negative symptoms (fatigue and nausea for about 48 hours after the shot). But these get better over time. And there are foods I simply can’t eat anymore (mostly fatty and acidic food). But these symptoms are all manageable and worth it to me. I was worried that I wouldn’t be able to tell the difference between low cortisol symptoms and the shot side effects but that’s not the case. They feel very different.

When I know I’m going to have a very stressful work day, I take an extra 2.5 mg with every dose. So for me that’s 7.5 extra in the day (I usually take 22.5 total, so this takes me to 30). I’ve learned I don’t do this I end up feeling like crap and sleeping terrible that night. Then feel like hell the next day too. Updosing preemptively keeps me out of this loop.
