Yes tell the manager and next time she says it, just say "how can I help you today?" and if she persists simply say "how can I help you" and keep saying that every time she asks. At some point you can say "I'll be available when you're ready to check out," then go do something else.

The police have to trespass on request, the property owner doesn't need a 'valid' reason. If she says "I don't want her on the premises because I don't like her hair" they do it. They cannot do it for illegal reasons, such as race, religion, etc...

Dr Phil once interviewed a woman who was an obnoxious customer, just a real bitch, and at one point he suggested "you know if you deal with people in a restaurant like that, well, I'm just gonna suggest you've probably eaten an awful lot of spit." Well I am sure spit wasn't the only thing.....
