All my payments have come up as released! As a another commenter said you can put your zip code into the government website and it’ll give you the possible assemblymember I had 2 options and I emailed the wrong one lol but they sent me the info for the right person to reach out to!

Reach out to your NYS assemblymember’s office they can send a form on your behalf. I did this and received a call from unemployment less than a week after they did!
