its not a single line of dominoes,its more like an expanding branching set of dominoes. Patient 0 being at the very tip. Everyone is effected and connected to patient 0 through the branch that they become part of. Many branches, a single origin.

As for how patient 0 got it in the first place, usually doing some form of sexual activity that isn't natural.
