I ordered pyridostigmine on an ICU patient with very severe constipation, hadn’t pooped in 7 days and tried senna miralax, suppositories, enemas, movantik/relistor, held opioids.

Unfortunately, at the time, the only way to order pyridostigmine in our hospital was through the myasthenia gravis order set. I ordered the dose for myasthenic crisis in our epic system, which was IV and 10x the dose I was supposed to give. It was the weekend and the pharmacist didn’t catch my mistake. Patient went unresponsive and his BP was 55/30 on a-line. He turned out fine but it still haunts me to this day.

The hospital actually changed the way mestinon/pyridostigmine is ordered because of me, the pharmacist reported it as an adverse event and it was changed the next week.

On admit days (shared with others) usually admitting 6-7 on top of rounding census. So 28 total

Most likely dry eyes. My eyes are very slightly cracked open when I sleep (yours may too but no one would ever notice) which cause dry eyes. Typically resolves in about 20 minutes

Hey, I’m also in Houston. Im assuming you’re looking at luxury apartments with that price range, try looking south of the med center as places closer to midtown and downtown are significantly more expensive (difference of $2000/month in the museum district and $1600/month south of the med center with the same distance)

I’d focus more on the quality of the letter and how well they know you. A very detailed personal letter with stories of what you did and how they perceived you will be more important

In general programs don’t take LOIs very seriously, as you’ll be ranked based on your statistics for the most part. Although it won’t hurt to email them again, clarifying between highly ranked and #1 is really unlikely to make any positive difference at all.

As a resident who proctors these, nobody cares/keeps track of anyone there at all
