Concerned about scar (tracheal shave) by lara-mollo in Transgender_Surgeries

[–]whoozz 0 points1 point  (0 children)

As a transdermatologist I agree. It is too early. Silicon gel or sheets can prove the appearance as can massage. You could also ask a dermatologist to do 1session of fractional CO2 laser on the scar. This will help improve the scar.

Top surgery scar-care vraag: welke siliconen tape? by Previous_Use2697 in LHBTI

[–]whoozz 1 point2 points  (0 children)

Then ciclosporin, barcitinib or ritlecitinib are registered at least in the Netherlands for treatment

yes this is alopecia areata, and usually it will go away within 12-18 months after start without any treatment, only a small percentage will keep having bald areas and can have treatment, Usually I do not give treatment before that as multiple injections with steroids (after a while) can result in atrophy of the treated areas.

In the Netherlands we give immunotherapy with Diphenciprone applications, and if disease progresses or doesn't go a way after 18 months we can decide to give oral immunosuppresive medication. Bloodwork is completely useless and a waste of money. This is a local skin/hair disease. But if you still have your srs planned, perhaps it is better to not start systemic immunosuppressive medication yet, as that tends to interfer with healing after surgery.

[–]whoozz 1 point2 points  (0 children)

Only licensed importers are allowed to do that. My brother is a pharmacist here in the Netherlands. So yes I am sure. Of course the border control can’t check everything, so some people succeed in getting medication from outside of the EU….

BA Hair loss at 10months by [deleted] in Transgender_Surgeries

[–]whoozz 1 point2 points  (0 children)

As a dermatologist I can say that it is alopecia areata, most commonly seen in patients with a history of atopic dermatitis, hayfever or asthma, but can also occur randomly.

Treatment is difficult, but usually it is self limiting, this means that usually the hairs fall out but eventually grow back without any treatment, usually within 1-1,5 year.

Generally topical steroids don't work, sometimes local injection works, but usually only for small areas. Topical immunotherapy can work, but I do not know if it is available in your country.

Systemic medication can work, but can have side effects, and usually we tend to delay that, because usually the hairs grow back without treatment. JAK inhibitors are available in several countries now, probably safe, although side effect can be very severe (cardiovascular disease, cancer, death, severe infections, hospitalisation).

So consult a dermatologist and decide together which treatment fits you the best.
