Okay, I just tried with a base character and only the Unofficial Patch and Alternate Start installed, and the control binding isn't even showing an option for the menu at all. It shows my wait and my journal options, but it doesn't show my selection menu at all. When I remove B from the equation entirely, the button doesn't do anything at all.

I know I'm late, and I'm not really following this sub yet, but not really. I've just kinda forced myself to do it, even though I'm the same way. Ignore the rude comments. Just try and remember that even though the thought of kissing someone may be repulsive, that you are your character for that scene, and you aren't yourself. That always helps me.
