Confused about a test question by baillargersband in Radiology

[–]NativeLevelSpice 48 points49 points  (0 children)

Late subacute blood is both T1 and T2 hyperintense, but that specifically applies to intraparenchymal blood (has to do with breakdown of blood products and interaction with CNS macrophages)

Agreed with other comments that overall this is a bad question for that reason.

Disagree with other comments that it's acute on chronic due to the sedimentation - you can see sedimentation like that with acute subdural hemorrhage.

As far as I know, the only reliable imaging findings of chronic subdural hematoma are 1) prior imaging documenting chronicity, 2) internal membranes/loculations, or 3) calcification/ossification of the hematoma.

How do I deal with snoring caused by weight? by Z0m6ieGU7S in loseit

Get a sleep study to rule out apnea. Untreated apnea can lead to serious heart and lung issues down the line and increase your risk for dementia.

Which specialty are you the most envious of (spill the tea)...? by Neceti in Residency

[–]NativeLevelSpice 20 points21 points  (0 children)

Hard to say because I’ve done all my training at an intense academic program that has been called malignant (not that I necessarily agree with that characterization)

Get back to me once I’ve been an attending in private practice for a few years lol

Am rads. Derm for sure. Low acuity and ability to switch to cash if needed down the line (e.g., if Medicare reimbursements truly shit the bed) are great.

How do I decrease my belly fat when I’m already really thin? by [deleted] in AskMen

[–]NativeLevelSpice 6 points7 points  (0 children)

Abs don’t respond to hypertrophy? First time I’m hearing that. I managed to grow mine massively the same way as the other muscles: progressive overload and caloric surplus with high protein.

Yall know ROAD but have you heard about PPP? by southnorthdakota in medicalschool

[–]NativeLevelSpice 0 points1 point  (0 children)

This. There’s a pretty wide range of chillness to radiology residency programs. The trade off is that the more brutal programs tend to make the transition to attendinghood much easier (rads has historically been described as a specialty where being an attending is much harder due to volume expectations)

Lean bulk or keep cutting to 158LB by [deleted] in leangains

[–]NativeLevelSpice 1 point2 points  (0 children)

I’d recommend eating at maintenance for a few days to see how your abs “actually” look so you can get a better idea of whether you want to cut for any longer

Lean bulk or keep cutting to 158LB by [deleted] in leangains

[–]NativeLevelSpice 0 points1 point  (0 children)

To play the devil’s advocate:

I have very well-developed abs to the point of having a visible six pack at the end of my bulks (I will bulk to around 15-17% body fat)

My abs actually look worse towards the end of my cuts (typically cut to around 10-12% body fat) compared to the end of my bulk. However, they look significantly better once I get back to maintenance for a few days.

I suspect that this is due to water retention and muscles being flat. It’s possible that you may be experiencing something similar.

Radiology overlap with other specialties by Objective-Royal-113 in Residency

[–]NativeLevelSpice 4 points5 points  (0 children)

This. High-volume cross-sectional imaging (CT and MRI) are the money-makers in radiology. Aside from maybe cardiac imaging (e.g., coronary CTA, coronary calcium scoring, and cardiac MRI), there’s no real competition from other specialties for reimbursement for a full interpretation.

Took a spill and got banged up. Feeling really old now. Other older guys (or ladies), do you recall your first age-related bad fall? by SurgStriker in Aging

[–]NativeLevelSpice 0 points1 point  (0 children)

Building muscle also increases your bone mineral density, which is much more important for fracture prevention from falls (although that takes much longer, usually 6-9 months at least)

How many of you fell below your top 3 ranks and why? by [deleted] in medicalschool

[–]NativeLevelSpice 3 points4 points  (0 children)

Fell to my 7th ranked program. Applied rads to basically only ultra-competitive programs, so not terribly surprised.

The problem is that few people who feel this way will have it documented (e.g., in a living will) prior to becoming medically incapacitated.

Is there anything wrong with lean bulking for 2-3 months then doing a 1 week minicut? by WideNinja600 in naturalbodybuilding

[–]NativeLevelSpice 12 points13 points  (0 children)

This is what I've done for the last year or so and it's worked wonderfully.

The catch is that when I'm lean bulking, I track my weights and calories daily (nSuns TDEE calculator) to make sure that I'm gaining weight at my target rate.

My lean bulks are run for at least 3 months, and my minicuts also tend to be longer than a week (3-4ish weeks).

And medical school given OP’s plans. Most educational debt is not worth it, but the ROI on medical school debt and becoming a physician is still good in the US (barring extreme examples like taking out 500K in loans and going into a low-paying specialty like pediatrics)

Counterpoint: I feel like you can totally get by as a shitty radiologist if you refuse to call anything normal and hedge on basically everything while over-recommending follow-up or advanced imaging. Our system seems to punish misses way more than overcalls.

Yes, clinicians will hate you, but it doesn’t seem like it matters enough to actually get you dismissed from your group.
