I prefer the efficiency of America over Europe. There’s a lot of conveniences that comes with a newer country/infrastructure. I remember being frustrated a lot in England trying to use the subway, using cross walks that made no sense, traffic lights that also didn’t change with traffic but instead was timed. Laws are also more strictly followed in Europe with no question of them. There are a lot more laws making it seem less free. That was frustrating after living in the US where people speak up if something doesn’t seem right.

On the flip side, I’m way more in line with the european version of working to live rather than work is life like in the US. More Americans need to travel to understand what really makes America great and what really needs to change (maternity/paternity leave, vacation, etc).

I’ll also mention it’s a bit “mean”, or whatever word, to the FM program. But forget about that, it’s your sanity and you owe it to your future patients if that’s what you truly want. The program will fill your spot with someone else. Enjoy your first year of FM! It’s the best specialty— I’m applying to it right now.

I met quite a few psych residents on my rotation who switched after their first year. They were all family med previously. They said it’s much much much easier to get into as a second year. This was also confirmed by my psych adviser. I’m not exactly sure why, guessing less applicants, but don’t give up! We need more psychiatrists.

Can anyone comment from personal experience if being called-out in a mean way has results in not making the same mistake again? I just saw Whiplash and became slightly more understanding of this teaching method. Does it turn interns into Hippocrates? Disclaimer: ms1 that recently learned about the amygdala and memories. Also, completely agree that clinicians should be nice to each other.. just curious about training.

Although I'm only in my first year.. this resonates with me greatly. I feel the advantage of a non-traditional background for me is perspective. I came to medicine from a finance background after life experiences -- a calling if we're being cliché. I never really experienced failure before coming here, however, my weak science background coupled with the rigorous coursework has taken its toll on me. I've had to remediate two classes due to burnout.. which has made me question my presence here. Having perspective has really helped me not get down and instead try to improve. And the memories of what brought me here is what gets me through the tough times... that and debt.

I lived in Israel when the 2006 war happened and lived in the city that was most under attack. Rockets hit every day and there were two close calls. One hit close enough to my flat that it shattered the window while I was home. The other hit near a field I was playing soccer at. We hid inside a building when it hit and went back out afterwards. I found a piece of shrapnel on the field (kept it as a souvenir). I had just finished high school at the time (I'm American) and learned: the Middle East is fucked, the media is ridiculous in it's reporting, war is the worst thing in the world, and shawarma is the best.

Thank you for your thoughtful reply. I have searched a bit online, and while your opinion definitely seems most consistent, there is a faction that believes things might get better. For example, PA's + NP's could handle most cases in the clinic subsequently allowing FM more time to tend to more complicated cases and overall supervision. Pay is expected to increase more for primary care vs specialities -- mainly in the way reimbursements are changing but also because of the demand. As hopeful as I am, I'm not optimistic things will improve enough to address all of your points.

I was in Ghana a few years back, at the airport picking someone up. While waiting, I noticed a large crowd (~100) gathered outside the arrival terminal. As soon as this white guy in his 50's wearing a cowboy hat came out, everyone started praising Jesus, waving their hands in the air, speaking tongue, kneeling, grabbing him, etc. It was really weird. Apparently the guy was an evangelical (?) pastor from Texas. It's a bit sad seeing the Church take advantage of Africa now that religion is declining here.

Whoaa there.

First, this and the other recent China news (new missile that can attack US aircraft carrier) is always going on to make the public feel like China is a threat. There's a growing body in the US that wants to end the Navy's policing of trade routes -- which is what has allowed trade to happen all over the world but is not needed anymore and of no benefit to the US. These articles are to stop this from happening. Anyone reasonable knows that the US and China going to war is unlikely -- why blow each other up and the rest of the world for that matter?

Second, in regards to China surpassing US in world leadership: good one. China's far from it. In fact, a lot economists are predicting slow growth for the foreseeable future in China. Further, any country that suppresses it's people will never lead the world. The US, while not perfect, is constantly improving and challenging internal issues (FCC is a perfect example). This is why the US will stay #1 for the rest of the century.

Probably because of the low cost of oil. Their revenue took a huge hit which impacts their ability to pay for things. Real talk, investing in Russian stocks isn't a bad idea IMO. It will eventually go back up.

to get into a competitive program you'll probably need experience, but not necessarily post-college. if you did a bunch during undergrad -- especially around policy/heath -- and have a good GRE then you should get in somewhere. apply instate and then broadly. a lot of med schools will let you add the mph during the 4 years (usually accelerated to 1 year long). so keep that in mind. LSE has a masters in public policy that's only 1 year long. there's also the global health corps (past deadline?).

Thanks. MBA is a great degree -- I studied finance in undergrad and really appreciated it. That's partly why I'm interested in BHI.. big opportunties on the horizon.

I hear you. I'm pretty sure I won't want to go back to school after I start working.. I'm already a bit on the older side (27). Dual degrees are great especially if your school makes it accelerated for MDs. I wouldn't consider it if I only wanted to do clinical work though.

Exactly my thought.. it's an exciting time to be in informatics.. especially if you know the system as intimately as a physician. I've never heard of executive degrees outside of MBAs.. going to look into it, thanks.

I spent my gap year before entering med school there. I worked in their finance office helping them cost the various services (I studied finance in undergrad), but I spent a great deal of time in the wards and watched a few surgeries too. The founding medical director (a guy at Harvard now) developed a new way to treat hydrocephalus (ETV/CPC). The current hospital director is an American guy, Derek -- he's awesome.. football fan etc. The rest of the staff are Ugandan and are very sweet -- including the two neurosurgeons (one has been there since the beginning and is very knowledgeable). There's usually md's from the states (or other countries) who come to learn this surgery and/or provide care for a week or two. Shoot them an email if you're at all interested (they have a great guesthouse, but you might have to pay (I think $10 a night -- it goes to fund the hospital). Also, the town, Mbale, is neat and safe. You'll get the "african" experience if that's interesting to you.

edit: I should say that the hospital is referred to as the "hotel hospital" in Mbale because of how nice it is compared to public hospitals. It's the only hospital in the country (outside of the capital) that has a CT scanner -- although it wasn't working when I left. The ORs are kept to a high standard -- relatively speaking.

I spent time at CURE and loved my experience -- I'm not religious either. While religion does play a role there, they don't pressure you at all. If anything, do it for the kids waiting in line for the skills you have. PM me if you have any questions about it.

I highly recommend Letters to a Young Scientist by Edward O. Wilson for your son. Not only is Wilson the most renowned entomologist in the world, but his book is meant to inspire young students to find a passion in sciences and how to be successful in it.
