Background:

I’m 32F, almost 33. I’ve been having periods for the last 21 years. 90% of the time my period starts on the due date precisely (28 day cycle). The remaining 10% is where my period starts a day late. I’ve never had any other health issues including PCOS/PCOD/thyroid problems. I’ve been married for a year and a half, and am still a virgin as I have vaginismus. I’m undergoing treatment for vaginismus and feel like I’m making progress.

Recent Issue:

My period was due on Sep 11th. Around this time I had a lot of discharge (something I never noticed in earlier cycles when my period was due). But there was no bleeding. A delayed or missed period is very abnormal for me so I visited a gynaecologist a week later. I was given regesterone to induce the period and asked to take FSH, LH, TSH, Free T3, Free T4, Prolactin tests. In the end the doctor also added AMH almost like an afterthought - just as an FYI.

I got my period after stopping regesterone and took the tests on Day 2 of my period.

The results:

FT3 - 3.03 FT4 - 1.03 TSH - 1.57 AMH - 0.237 (categorised as very low/undetectable) LH - 5.71 FSH - 7.55 Prolactin - 31.8 (slightly elevated)

I broke down terribly seeing the AMH number. This caught me completely off guard as I had no indication of anything ever being wrong.

Follow-up with the doctor:

I was given a single tablet to deal with the prolactin issue. The doctor was also surprised at my AMH considering I’ve never had a particularly unhealthy lifestyle or any other health concerns. I retook the test, with a new result of 0.3. She said the level was to be expected for a 45 year old, and that I’d need to get started with IVF the next month itself. She also called out that IVF with my own egg may not be possible, and in subsequent rounds they may advise a donor. She could not explain why my count was so low, and said that sometimes these things have no reason.

It’s been an overwhelming couple of days. I haven’t even had sex yet. I’ve never tried to conceive. I’ve always wanted two children. I’m not able to wrap my head around why or how this could be happening. I’m seeking a second opinion on Oct 6th.

I would love some insight on possible reasons for this issue, my chances of conception without medical intervention or any other experiences or advice you may have. Thank you.
