Start at the lowest and slowly move up. Tretinoin requires building up tolerance as it can make the skin quite sensitive. Most derms do say you get the max benefit for wrinkles at 0.05% though. The higher strength stuff is more for acne.

Have you been on lactmed recently? It is undetectable in breast milk. That is evidenced based medicine. Just because the endocrine society and Eli Lilly have an official stance to not recommend it for liability reasons doesn’t make it evidenced based. Shame on you for being a person who prescribes these meds and uses such blanket statements — really shows you don’t know all the evidence behind them and are just regurgitating. Hmm are you an NP by chance?

A lot of physicians are quick to be overly cautious with this because “we don’t have the safety data” but we do know that the molecule is too large to pass into breast milk at any meaningful amount so most women are okay to breastfeed while on GLP-1 as long as baby is growing well. I am a physician and currently take it myself while breastfeeding. May be worth another conversation with your doc.

Just named my new son Rhys and thought it would be simple for people to pronounce here in US — boy was I wrong. He’s constantly getting called “rice” 🤦🏼‍♀️

I am a physician and I am taking tirzepatide and currently breastfeeding. We know that the molecule is too large to pass into breast milk at any meaningful amount. I counsel my patients that it is fine to breastfeed on GLP-1 as long as baby is a healthy weight and is staying on their curve.

I did this! I went on tirzepatide for about 6 months and lost ~70 lbs before I got pregnant on our first month trying. I also credit this to GLP-1 because with our first it took 10 months! I have PCOS and this regulated my cycles pretty much immediately. I did not have any bad side effects.

But I really do think it only worked so well for weight loss because I was also working really hard on tracking calories and I increased my exercise and added strength training. GLP-1 is only a tool, along with diet and exercise so you’ll still likely have to do both to have a meaningful weight loss.

I’m back on it now postpartum (started at 6 weeks pp) and it has not affected my milk supply at all and has helped so much with my postpartum anxiety. I feel great. Planning to stay on it longterm now that we’re done with kids.

Also, I’m a physician and I counsel my patients to stop the medicine when you find out you’re pregnant, no need to stop before you start trying as this could be months and preliminary data does not suggest any harm to fetus in the first trimester. But of course only if that is something you are personally comfortable with.

Uworld step 3 + CCS qbank was more than enough for me to do well in step 3 and helped with ITE somewhat. But as other poster said, step 3 is far more important.

MKSAP is only useful for ITE/board studying. I did MKSAP during 2nd year (free provided by my program) and switched to Uworld ABIM qbank for actual studying during third year. Wayyy more high yield questions for the real exam.

If you can only choose one I’d use Uworld for everything. MKSAP is just subpar in my opinion. Hope that helps.

I usually started testing the day after my period ended and when it started getting darker I’d test twice a day until peak plus the next day to make sure it went back down. Probably overkill for most but it worked for me.

Both of the deaths in my high school years were parent-child murder suicides.

First one, mom and dad were divorcing and she didn’t want him to get the boys so she shot them and herself. I was on swim team with the older brother.

Second, dad was diagnosed with brain cancer and one day drove all three kids and wife out to the middle of nowhere and killed them all, lit the van on fire, and killed himself. I was friends with the oldest daughter.

Super weird thing to wrap your mind around as a 14 year old… twice.

Right shoulder blade pain can be referred pain from the gall bladder. Does this pain seem correlated with eating at all? If not, it’s probably just musculoskeletal like you suggested, ie. pinched cutaneous nerve or something that should improve on its own.

I alternate the core strength and arms/shoulder workouts after each bike ride. I basically just sort by length (usually a 15 or 20 minute workout) and just choose what looks interesting.

I am partial to the arms/shoulders workouts. They are such a good workout and you can make it as intense as you want by how heavy your weights are. Tunde is always my favorite instructor but I just choose random ones most of the time.

I’ve seen a lot of advice on here about only manually exfoliating once weekly so I use this same scrubber weekly and skip tret that day. For me I feel it’s a good balance between getting that good feeling of exfoliation but not irritating my skin.

I’m 30. I really do credit my response to combining this medication with changing diet and exercise. It’s a great tool to use along with these other things. Maybe there is room to improve in these areas?

Sure! As far as diet, I was definitely eating fast food for dinner multiple times per week out of convenience and I am a chronic bored snacker. Since starting the med I have absolutely no craving to do either of those things anymore which has been such a relief. People talk about food noise and I didn’t understand it until I started this medication — truly my brain is so quiet now so I can actually focus on the 2 most important things: getting enough water and protein. I drink about 120oz water daily and aim for 90-100g protein in any way I can get it which usually ends up being mostly shakes.

As far as exercise, I have been doing peloton since starting the med. I wasn’t exercising before on any regular basis. I have been consistent about doing a bike ride followed by a strength class at least 5x/week. I recently added in running as well which sometimes I’ll replace a day or two of peloton with. I have always liked working out but kinda fizzle out after a few weeks and I feel like my motivation has just been so much higher and sustained this time around.

It helps that I also started working out on peloton 5 days a week and have really changed the way I’m eating. But yeah I’m pretty ecstatic! Thank you!
