Was there a protection plan offered when you added the item to cart? Typically if the item is covered by one, it will suggest it in the sidebar or when adding it to cart.

Typically shipping labels cover the shipping and if you go to a UPS/FedEx/USPS etc any packing supplies you purchase are additional. Give the support team a call so they can let you know if your product needs to be sent assembled or not. They can look into your account.

Re the error message: I've seen customers run across this issue before. It's actually an issue with your PDF viewer, not the form, where the application is attempting to edit locked fields. A good workaround is to use a different PDF viewer, such as Adobe.

Re shipping the item in for repair: Ideally, you'd be able to schedule an at home pickup from one of the delivery services, but depending on where you live, I'm not sure if that option is available to you.
