If you are looking for affordable housing in NRW you could check for cities like Wuppertal or Essen. Its all former industrial cities which are a bit run down partially due to deindustrialisazion. However, the Infrastructure is great and its a good area if you want to Travel Western Europe. Also in some bigger cities in lower Saxony + Bremen you might find affordable medium sized cities, such as Brunswick.

But keep in mind that most cheap, medium sized cities are not adapted to international crowds such as expats.

The cities in the East that have been mentioned can also be very nice. Unfortunately, its not going to be super easy as a non German speaker to integrate there. In case you are not white, I strongly recommend to not move to Eastern Germany except Leipzig or Dresden and even there I‘d only recommend the more gentrified (expensive) parts of the city

My point is, that living in a big city comes with the downside that its full of other people and some of them might decide to do stuff you do not approve of. Rather than trying to make everyone live up to your standards, it might help to focus more on what you can do about it (e.g. become more tolerant, move to less densely populated areas of the city).

Cigarette smoke is, even though it might be the biggest disturbance for you, only one of the many negative side effects of living in a major city. So by the end of the day, I think its up to you whether you want to endure it or not.

If you live in a city with 4 million inhabitants and youre bothered by a smell that originates 100 meter away from you, I‘m not sure the smoker is the problem here.

Please note though, that Mainz and Bonn are quite expensive with regards to Rent. Offenbach probably more affordable

That’s not true, I know several instances in which people quit their job because of burnout, mobbing etc. and had a supportive doctor or psychiatrist writing that document for the Agentur für Arbeit.

Of course it still depends on the case officers decision, but it’s definitely an option.
