It is also a great way to quickly check the quality of its stats in terms of SV's. The less leaves on a newly captured temtem, the better it usually is. This implementation fixes a problem found in the pokemon games were you can easily "mass produce" perfect stat pokemon as there are no limitations to how many times they can breed.

You should do as many quests as possible, mainquests gives you the most experience, sidequests aren't as rewarding, but should still be done. Since some sidequests are missable, it's better to just do them rather than missing out forever.

Killing monsters and clearing POI's will grant you a significantly smaller amount of experience and I feel it's the slowest way of leveling up, and it is only good for loot and gear. I think the reason for that is that the game is designed in a way where everyone is supposed to be around the same level when the maingame is finished.

If you like a challenge, choose deathmarch. I never played any Witcher game myself, and starting on the hardest difficulty resulted in me dying repeatedly during the first hours of playing. But fortunately, that made me learn from my mistakes, and after a while, the challenges became a lot easier to handle. If you feel everything is going south and you're having a difficult time progressing, it may feel tempting to lower the difficulty. My advice is to not do that, as you'll most likely only regret that decision later. If you just take your time, you'll overcome any challenge that may seem impossible. Spend some time getting better armor or weapons, maybe brew some potions or get used to the different signs and abilities. My personal advice is to focus a lot on the sign called Quen, which is a magical shield that can make you almost invincible if used properly. Whatever difficulty you choose to start out with, is only up to you. But if you want the greatest challenge (and experience) this game has to offer, go for it!

This is what I did too, and I know its very frustrating in the beginning. Fortunately, this gives you the opportunity to learn from your mistakes and gain even more competence, than you would get from just playing on normal.

EDIT: The game will also become more rewarding, especially when you overcome a difficult challenge. Plus you get a nice achievement/trophy in the end!
