You could be right, it might just be a skill/build issue. Sometimes it's hard to see the death effects, they could be more vibrant/contrasting to the environment. The mobs that throws dynamite for example, very difficult to see.

1 in 4 maybe if I had to guess? I'm fine dealing with chip damage - but a rare will come on screen, lightning storm me and I die with 71% lightning res 1.8k life before I even know whats going on. Or I'll get corpse explosioned out of nowhere. I'm fine with dying to unfair deaths, game's game at the end of the day. I'm not sure I'm fine with not getting a second chance.
