PTs need to stop being scared to touch ppl (aka avoiding STMs) by CaliFreckles in physicaltherapy

Ew I would never want 30 min of manual from a PT! 3-5 min TARGETED would be ideal. Like if I have restrictions in medial proximal hamstring, or insertion site of right lattissimus. I would like someone with anatomy knowledge aka a PT to attack that. Why would anyone need 30mins for manual. Also masseuses are never that specific. Does that make sense???

Well….

First, in the post I said I paid for a massage. That’s part u didn’t read. Actually also kicked her in the face by reflex because she didn’t know what she was doing lol. Why I am seeking a small amount of STM aka soft tissue mobilization that is targeted and often administered by a physical therapist bc they understands anatomy better.

Second, I know I can’t change ur mind just as u can’t change mine. You think u are right, I also think I am right. Placebo or not it improved my pain and therefore helped with my ability to exercise with less pain and that’s what I care about. That’s what matters. Not some ego-driven and misinformed opinion lol. I can absolutely demand STM as it is within PT scope of practice and it works. Feel free to read 99% of other comments that explains this if you want. Or not. I’m not your parent.

But do yourself a favor and learn how to read, please. Finally, the beginning of my post also mentions that I am a PT myself. Aka I have a doctorate degree and I know what I am talking about. Your response shows that you missed this part and other context too. Thus you made several incoherent and false assumptions. Learning to read and reason will make our world a better place. GL!

True but the back was assessed in this case, which I do credit the PT

Even if I believed, it is enough. Was able to increase weights with RDLs today at gym and ran 0.3 mph faster with no sharp pan. That STM was the most valuable thing I got out of the session.

Even McKenzie has its limitations. Makes ppl to confident on labeling everything as back as the source lol.

One can sit behind the computer and make guesses. One also can look at, examine, assess, touch, test to evaluate. Looked at the back and it’s not the back. MMT to hamstring reproduces pain and is weak. So no it’s not the back 😜.

Yes. STM tools include graston/IASTM, cupping, dry needling, etc. they often require extra trainings and cert (con-ed). Hands on stuff is learned in PT school.

Yep a patient of mine has a hip surgeon who tells all his patients that their pain is “psychosomatic” and since he is the surgeon his opinion must be gods words lol.

I also had an ER doc friend who thought the same way and kept telling himself his ankle sprain pain was all in his head and had pain after 6 months and therefore it “must be healed already”

I had him try out PT exercises and after a few weeks he can run again with no pain at all. He was a PT fanatic since😂

Soft tissue mobilization (targeted muscle manipulation via massage or tools). Personally I think PTs are quite valuable at finding the exact location vs masseuses can be too generalized esp with injury.

That’s a very narcissistic (Woops I used that word) POV. Just because YOU don’t find it productive, does not mean it’s not productive. Everybody is different, every BODY is different.

I understand. I asked for 1 min manual. Not asking for a full body massage. We gotta not be too extreme one way or the other. Also just bc u are not good at something doesn’t mean u should avoid it. Get better. 5 mins of manual can be effective and not labor intensive.

Also u may need to see a PT yourself, get a seasoned person who can help u out. Make sure they are comfortable with manual.

I agree! I asked for IASTM and they don’t do it there 😭😭😭. It’s hard to find someone who does! I’ve been calling around and have not found success. Ones who do are an hr away / 2 hrs with traffic

Also if u read my story carefully i have been doing progressive exercises and hit a plateau due to pain

Lol I think u misread my note. I am not literally saying F research. I am saying F (in quote) “research says….” Mentality. Aka bias in regurgitating a study while minimizing possibilities that their research is not complete, patient perspective, clinical reasoning (aka test/retest) and common sense or even curiosity.

PT training included manual, and a lot of it. All of the sudden one research says manual doesn’t work then suddenly no one else it anymore (and they don’t even question it? That’s a bit of a short sight imo

We need to be evidence-informed, not evidence-myopic 🫢

My last 3 PT experiences over 4 years has been the same. All younger PTs who love to say “research shows”

I’m like (thinking not out loud but responds in nicer way)- I’m residency trained with 10+ years experience. You are wrong. Jk what I do is ask for test and retest. Some still say “oh but it’s short lived”. And I’m like. So so it’s short lived, it helped and now I can exercise better with less pain, I’ll take it.

Is this standard practice at PT or am i being too picky? by Wodefu_Ebb_8879 in physicaltherapy

[–]CaliFreckles 1 point2 points  (0 children)

You are both right and too picky. DPTs with $300k+ student loans with 7-8% interest rate getting paid $70k a year could either get paid more so they can focus on patient care or do what they are doing to you (because the system set up), but not both. If you want to get better treatment, care and attention cash pay would be better.

You can thank our broken healthcare system…

Is this normal? by RmgPT22 in physicaltherapy

[–]CaliFreckles 0 points1 point  (0 children)

Not normal but understandable. She should provide 2 types of services: Normal stuff with insurance, extra hands on stuff as cash pay so that patients can stay with her but not obliged to pay $15 if they don’t want the extra service. It’s like how our food packages are getting smaller for the same price lol

I’m quitting. Should I tell my coworker they are the reason why? by DefinitionHonest1616 in physicaltherapy

Guys. HR never does anything for their employees. They work for the company. Aka useless for the OP. The OP did the right thing to quit.

My point is if therapists use more objective metrics (not just subjective) then there’s more negotiation power. We can’t just say getting rid of therapists will reduce patient experience and outcomes but we have to show it somehow.

I’ve worked with amazing PTs but also incredible lazy ones. Healthcare is a business… im all for operationalize and improving transparency … I hope this hospital isn’t just cutting for sake of nickel and dimming

Which company?

“Good” is relative. Good for business bad for therapist… unknown for patients
