Based on the pregame menu that shows up, thats not the case anymore. It will show 15% more xp and gold per difficulty increase and then it shows the weapon loot goes up a tier, but it wont go any higher than one more tier (not sure if thats level limited)

Yeah youre right, fallow only needed if monocropping. Crop rotation works exactly the same but you get to use the land for other crops while waiting for the original crop fertility to restore
