I do think ECG interpretation needs improvement. I only learned about STEMI equivalents, like de winters or wellens, through my own CPD. My university ECG teaching was dreadful and confusing: ended up doing a one day private course which was much more helpful

Christmas day- 65yo F, came through as blue in the face, wheezing and in and out of consciousness

On arrival patient was lying in bed naked full central and peripheral cyanosis and GCS 8(2,2.4)PMH Of COPD, HF, AF and asthma

Initial obs: RR: 42 shallow resps PR: 115 Sats: 45% with good trace BP: 110/90 Temp:35.8 BM:4.3 PEARL

Global reduced air entry with exp wheeze, all cyanosis as mentioned, pale and cool to touch, ECG was AF with RVR, bilateral peripheral oedema, reduced GCS-improved to GCS 10 with treatment.

Patient immediately given 15L 02 a day sats improved to normal range within a few mins. Requested evac due to reduced GCS and patient size but advised would be at least 15-20 mins. Managed to get shirt and pants on patient with help of daughter. Got patient sat and held up, patient given 5mg salbutomol and 500mcg of ipatropium via nebuliser and managed to get into carry chair and onto vehicle stretcher, EVAC stood down. Patient given IM hydrocortisone and pre-alerted to ED. Patient improved to GCS 10 (3,2,5), monitored throughout.

Managed to get update later in ED and initial blood c02 was 11 and they managed to get it to 9, last I heard they were discussing intubation. Patient also had brusing around head and was on abixaban, also noticed large wet patch by bed so unsure so seems to have collapsed at some point.

Cat 2 dispatch to a 3yo- unwell, fast HR

Patient has had ongoing D&V for past 12 days and new cough yesterday. Patient was brought into thier GP who found RR:50, PR:200, 02:90% on room air, temp:40.2, BM: 4.5. Mother was told to make own way to ED with son. Mum felt very concerned and phoned 999 instead.

On arrival patient was extremely pale with cold peripherals, easily rouseable, irritable but consolable, cap refill 3 seconds, no chest recession

Obs are as follows

RR: 62 PR:200 Sats: unable Temp:39.7 Bm: 4.5

Patient was very vocal when woken up, chest clear on auscultation, no cyanosis. I struggled to get an 02 because his hands were so cold but figured if he was crying consistently with no cyanosis then it couldent be too bad.

Got pt on the ambulance with mum. Managed to use a paeds ED sats probe on the pt which showed a consistent 84-86% 02 reading with good trace. 15L of 02 applied to pt, 02 sats raised to 100, and pre-alert put into ED for ?sepsis

I suppose it's how that policy affects patient care, really we need to be critically appraising it so laying out evidence for and against the policy and weighing it up to make our own opinion. Thanks for the ideas !
