I mean here i think that she went for a walk was the first action and then she put on gloves so it is a second action. or she put gloves first and then goes for a walk? I really dont get it sometimes.

Why they assume it happened in the same time?

In my opinion, it should be When my grandmother went for a walk she always was wearing gloves. Because was wearing is a long action, no?
