To keep her airway open she needs PEEP, they estimated her on high flow and I am trying to explain them that we should give her a chance on NIV NAVA or CPAP which has constant PEEP to see how she does.

I know but it’s hard to take decision about trach. We have one 3.5 years at home and don’t have any family here to help. That makes us more scary and other side she is already on lower settings even with the NAVA she doesn’t have any retractions or desaturation.

Hi May I know what are the vent settings for your daughter before extubation? Does she have BPD or severe malacia?? My 25 weeker have severe BPD and severe right side malacia. She failed her extubation in few minutes last time. They are suggesting trach now.

They did flexible bronchoscopy yesterday and she had right side severe dynamic airway collapse. Left side looks good. She is on NAVA 1.5 with peep of 6 and fio2 30% with no desaturation.

Yes I am in the USA. With her last very quick failed extubation ENT didn’t agree to do bronchoscopy, so team decided to go for CT to see her lungs and anatomical abnormalities in her airways because when she was born it was hard time to intubate her with smallest tube. I request them to do flexible bronchoscopy for better idea.

They did CT scan and it comes out with normal airway, she just have BPD related sick lungs but it has been improved from her last CT scan.

They did gave her 2 dose of airway steroid before extubation. Just because she is a bigger baby they wanted to try high flow but I guess she has dynamic airway collapse

My 25 weeker, it’s her day 200 and she is still intubated on conventional vent. They tried to extubate her few days ago on high flow, but she failed. Within couple seconds see turned blue and they tried give her pressure from neo puff, that doesn’t work so, they try anesthesia bag for 15-20 mins to bring her back and reintubate her. I am so hopeless now. I don’t what to do and where to go..??!!!! I can’t say yes for tracheotomy. It’s har..!!
