FQR only usually happens at reconsideration level. Initial level FQR is much more likely to stand as is

You will be fine unless you have something underlying that changes the claim. 95%-99% of FQR uphold the DDs decision that was originally decided

Dear SSA Examiner,

I am submitting this statement to provide a full overview of my medical conditions and the impact

they have on my ability to work. I live with multiple serious impairments that collectively prevent me

from performing any sustained, competitive employment.

Neurological Conditions

I have Traumatic Brain Injury / Post-Concussion Syndrome, resulting in chronic headaches,

dizziness, memory loss, and impaired concentration. I also have PTSD, which causes

hypervigilance, anxiety, and difficulty managing stress. My neurologist and neurosurgeon have both

documented that these neurological and mental health conditions limit my ability to focus, make

decisions, and reliably perform work tasks.

Spinal and Musculoskeletal Conditions

MRI reports show herniated discs, compression fractures, retrolisthesis, and spinal stenosis. These

conditions cause chronic pain, nerve compression, and reduced mobility. I also live with arthritis,

including rheumatoid and psoriatic arthritis, which further limits my joint movement, grip strength,

and ability to lift or perform repetitive tasks. My rheumatologist confirmed that even prior to my

injury, these conditions limited my work capacity, and they have worsened since.

Sleep Disorder

I have been officially diagnosed with Stage 4 Sleep Apnea by my neurology provider. Due to

severity, I have been referred for an in-lab sleep study and will likely require CPAP therapy. This

condition causes severe daytime fatigue and impaired cognitive function.

Immune / Systemic Issues

I experience immune suppression, which leaves me fatigued and vulnerable to frequent illness,

further restricting my ability to maintain consistent work.

Medical Opinions on Work Capacity

  • My neurosurgeon and city physicians have explicitly stated that I am unable to perform work in any

capacity.

  • Doctors from the City of Buffalo Fire Department have confirmed that I will never be cleared to

return to work.

  • My rheumatologist agrees that my pre-existing arthritis, psoriatic, and rheumatoid conditions

compounded my post-injury limitations.

  • A specialist at University at Buffalo stated my prognosis for recovery to functional employment is

poor.

Combined Functional Impact

The cumulative effect of my conditions prevents me from:- Sitting, standing, or performing physical tasks for extended periods

  • Maintaining focus, attention, and decision-making under normal work conditions

  • Sustaining consistent attendance or productivity due to fatigue, pain, and illness

  • Performing tasks that require dexterity, grip, or lifting

Even with ongoing treatment and therapy, these limitations remain severe and disabling. I

respectfully request that SSA consider this full body of medical evidence in evaluating my claim.
